This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. The responses received from interested contractors will assist the Government in determining the appropriate acquisition method. The Department of Veterans Affairs (VA), NCO 21- is conducting a market research survey to identify potential qualified sources capable of providing a A1C Hemoglobin High Performance Liquid Chromatography (HPLC) Analyzer, Reagents and Supplies Cost Per Test for a fixed price for a 5 year ordering period indefinite delivery/ indefinite quantity (IDIQ) or blanket purchase agreement at VA Northern California Health Care System. *****Contractor must be able to meet all requirements of the attached Statement of Work.***** RESPONSES MUST INCLUDE THE FOLLOWING INFORMATION: Company Name, Address, Size Standard, DUNS #, Cage Code Point of Contact name, phone number, and email address NAICS Code 334516 and Small Business Size Standard 1,000 employees Statement of Capability that demonstrates the offeror's past performance in providing this type of supplies. Please include the following: (a) documentation showing conformance with FDA regulations (FDA approval) (b) examples of prior completed Government contracts, references, and other related information (c) documentation explaining how the Statement of Work will be met. *NOTE Acquisitions will only be set aside for SDVOSBs/VOSBs if two or more responses are received and meet the non-manufacturer rule with the required information listed above, by the due date specified below. Responses are due to Moneque Rodriguez at moneque.rodriguez@va.gov by 3:00 PM (Hawaii Time) on Friday, June 04, 2021. No submissions will be accepted after this date and time. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. Respondents are solely responsible for all expenses associated with responding to this RFI.
