The purpose of this Sources Sought announcement is for market research to make appropriate acquisition decisions. It is neither a solicitation announcement nor a request for proposals or quotes. Description: The Department of Veterans Affairs, Network Contracting Office 21, is seeking potential sources to provide Community Nursing Home (CNH) services within the VA Northern California Health Care System (VANCHCS)- Rancho Cordova, CA. Nursing home care shall meet the total medical, nursing, and psychosocial needs of VA beneficiaries and must: 1. Have current Center for Medicare and Medicaid Services certification 2. Be licensed by the State of California The VA Northern California Health Care System (VANCHCS) serves the following California counties: Alameda, Amador, Butte, Colusa, Contra Costa, El Dorado, Glenn, Placer, Sacramento, Shasta, Siskiyou, Solano, Sutter, Tehama, Trinity, Yolo, and Yuba. The proposed NAICS Code is 623110, Nursing Care Facilities (Skilled Nursing Facilities) and the corresponding small business size is $30 Million. All capable businesses are encouraged to reply. Responses shall include: Business Name and Mailing Address; Point(s) of Contact Name, Telephone number(s) & E-mail Address(es); DUNS and NAICS code Business Size (small or large) and Business type (Veteran Owned, HUB Zone, Woman-owned, etc.) Pamphlet describing nursing home (location, type of care provided, etc.). The pamphlet shall not exceed 2MB or 5 pages. Answers the following questions: a. Is the establishment licensed by the state of California? b. Is the establishment certified by the Center for Medicare and Medicaid Services? c. What is the firm s level of experience with case-mix reimbursement under a Federal rate structure (e.g., Resource Utilization Group)? d. Does the establishment meet all state requirements for level of medical liability insurance? e. Is the establishment compliant with the requirements of 42 CFR 483.35? f. What is the number of beds available for Veterans? g. What is the facility s CMS Star Rating? h. Does facility provide any specialty services such as vent/trach capability and respiratory therapy?
