The purpose of this Sources Sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service-Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, and other Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Vendor must provide a capability statement that addresses the company s qualifications and ability to perform as a contractor for the work described below. The VA Pacific Islands Health Care System (VAPIHCS) located at Honolulu, HI is seeking a qualified contractor that can provide respiratory protection training services. All professionals providing services under the terms of this contract must be appropriately certified and possess the skills and experience to perform the services as required. The NAICS code for this acquisition is 611430 ($12M). All offerors must be registered in the System for Award Management (SAM) at www.sam.gov. All offerors should submit their information by email to Jocelyn.layson-castillo@va.gov. Submissions should be received no later than Monday, April 27, 2020 by 3:00pm HST. (See draft Statement of Work). STATEMENT OF WORK Respiratory Protection / Respirator Training 1. REQUIREMENTS: SOW - The contractor shall provide per this statement of work a complete turn-key respiratory protection training services beginning September 1, 2020 through August 31, 2021 to include all associated costs; such as instructors/trainers, office supplies, computers, testing equipment, transportation, fuel surcharges, food, and lodging, necessary to provide onsite initial and reoccurring annual fit test and training with an N-95 Half-Mask, Half-Mask elastomeric and Powered Air-Purifying Respirator (PAPR) Hood to initially train and fit-test approximately 200 healthcare personnel assigned to patient care, home base care, industrial areas personnel, and several VA Community Based Outpatient Clinics (CBOCs) at the VA Pacific Island Health Care System (VAPIHCS) to meet compliance requirements set forth in OSHA 1910.134. Option Year 1: September 1, 2021 through August 31, 2022 Option Year 2: September 1, 2022 through August 31, 2023 Option Year 3: September 1, 2023 through August 31, 2024 2. CONTRACTOR'S RESPONSIBILITIES: In accomplishing any work under this contract the contractor is totally responsible for his/her full compliance with any and all applicable, federal, state, and local government policies, regulations, procedures, specifications, etc., that apply to work of this nature. The contractors shall ensure the Program Administrator is sent electronic documentation in Microsoft Excel or Access, that will categorize and track training annually of healthcare personnel who have been fit-tested and trained that contains as a minimum: (a) Date Tested; (b) Assigned Care Line/Service; (c) Name of employee tested; Occupation; last four Social Security Number (d) Fitted respirator make, model, style, size, and filter type/number, if applicable; (e) Type of fit test performed, i.e. QLNT pr QNFT; (f) Pass/Fail test results if QLNT or Fit Factor test results if QNFT. The C&A requirements do not apply, and a Security Accreditation Package is not required. 3. CONTRACTOR'S WORK HOURS Ambulatory Care Clinic and Community Living Center: All worked performed at the VAPIHCS shall be between the hours of 0700am and 1600 hours, Monday thru Friday except Holidays. Contracting Officer's Technical Representative (COTR) in coordination with the Contractor will schedule shift work and testing times to accommodate personnel shift changes in early morning, and nights on mutually agreed upon dates and times upon receiving an initial request two business days prior to the requested training date. Contractor shall provide the names of employees and the applicable contact information (i.e., phone numbers and emails, etc.) to the COTR, that are scheduled to work at any VAPIHCS site at least 48 hours before arriving on VAPIHCS premises. The frequency of training will be required at least once a year. 4. CONTRACTOR WORK HOURS Community Based Outpatient Clinics (CBOCs): Onsite respiratory fit test and training shall be scheduled and performed between the hours of 0800am and 1600 hours, Monday thru Friday except Holidays, as needed. Contractors will provide the contracted services to VAPIHCS clinical personnel at Kauai, Maui, Hilo, Kona, and American Samoa CBOCs. The VA COTR and contractor will schedule, coordinate and complete additional requests for training within 10-30 business days (including weekends) upon receiving a request for training. The frequency of training will be required at least once a year. 5. RESPIRATOR FIT-TEST TRAINING: Contractor shall provide training to two major categories of respirator users at this facility (Involuntary/Mandatory and Voluntary/Not Required-Appendix D), which as a minimum must include: (a) Why the respirator is necessary and how to improper fit, use, or maintenance can compromise the protective effect of the respirator; (b) Limitations and capabilities of the respirator; (c) How to inspect, put on and remove, use and check the seals of the respirator; (d) Procedures for maintenance and storage; (e) Recognition of medical signs and symptoms that may limit or prevent effective use; (f) General requirements of the respirator standard 29 CFR 134; 6. REFERENCES: Contractor shall furnish at least three (3) references of current customers of similar or larger size and complexity to that of VAPIHCS. The references should be either in the State of Hawaii or other state, preferably within the healthcare industry. 7. CONTRACTOR ASSESSMENT OF TEST SUBJECT: Contractor shall ensure an assessment of comfort and fit of the respirator is performed on each test subject and that the test subject is allowed adequate time to determine the comfort of the respirator using the following points of view: (a) Position of respirator on the nose; (b) Room for eye protection; (c) Room to talk; (d) Position of the respirator on the face and cheeks; (e) Chin properly placed; (f) Adequate strap tension, not overly tightened (g) Fit across the bridge of the nose; (h) Respirator of proper size to span distance from nose to chin; (i) Tendency of respirator to slip; 8. CONTRACTOR FIT-TEST ASSESSMENT FOR TEST SUBJECT: Contractor shall use an OSHA-Accepted Fit Test Protocol (i.e., QLFT or QNFT), per OSHA 1910.134 for each test subject fitted, tested and issued a NIOSH Approved respiratory device. Contractor shall ensure the test subject conducts a user seal check, either the negative and positive pressure seal check as described in OSHA Standard Appendix B-1 or those recommended by the respirator manufacturer which provides equivalent protection to the procedures in Appendix B-1. The Contractor shall discontinue or not fit test a test subject, if any of the following are detected: (a) Hair growth, such as stubble beard growth, beard, mustache or sideburns between the skin and respirator face-piece that prevents or interferes with respirator face seal; (b) Any apparel which interferes with a satisfactory fit; (c) Test subject exhibits difficulty in breathing during the tests, she or he shall be referred to a physician or other licensed healthcare professional (i.e., Occupational Health Clinic), as applicable, to determine whether the test subject can wear a respirator while performing her or his duties; (d) If the employee finds the fit of the respirator unacceptable, the test subject shall be given the opportunity to select a different respirator from the approve list of respirators identified within the statement of work, and retested; 9. CONTRACTOR PERFORMANCE MEASURES: (a) Contractor fails to certify and provide a detailed work and training history of all trainers/training personnel that will work onsite at any VAPIHCS facility or facilities: (b) Contractor fails to maintain and provide copies of certificates or proof of having received at least the appropriate level of respirator protection training or experience for a program administrator in accordance with the program s level of complexity, as outlined in OSHA s Small Entity Compliance Guide 1998. (c) Contractor fails to provide and show up for scheduled training on any three (3) occasions during the contract period. 10. QUALITY CONTROL: The contractor should address the types and content of initial and on-going training, frequency of training, trainer qualifications, and training program regulatory compliance. Personnel will be required to show and wear identification badges at all time while on VAPIHCS premises. 11. INVOICE AND CONTRACTOR PAYMENT INSTRUCTIONS: The contractor will be required to provide electronic invoices for work/services completed via email or by fax, when email service is unavailable. Invoices shall separately itemize the following categories of charges/credits: (a) Labor: (1) Travel Hours (2) On Site Work Hours (b) Number of Employees Successfully Fitted Trained (c) Equipment (5) Materials Transportation (7) Lodging (8) Use of VA Equipment 12. LAWS AND REGULATIONS: The contractor will comply with all applicable OSHA, EPA, 49 CFR (DOT), and all other federal, state, and local laws and regulations. 13. PROOF OF BUSINESS LICENSES: Contractor is responsible for acquisition of all applicable business licenses and permits required by laws applicable for the State of Hawaii. Contractor shall certify that it acknowledges and is in possession of all required business licenses and permits. 14. APPROVED RESPIRATORS: The Approved Respirators List is representative of typical respiratory devices used by wearers of a respiratory device in this facility are listed below. VAPIHCS will provide a list of employees and the location for the training to the Contractor per paragraph 2 and 3 of the contract specifications. 17. VAPIHCS will supply PAPR and North respirators to the Contractor for fitting and trainig. The 3M 1860 N95 Disposable Particulate respirator will be used during the testing as the primary respiratory device of choice for fitting and testing all healthcare personnel or as required in paragraph 8 (d) of this specification.
