The associated North American Industrial Classification System (NAICS) code for this procurement is 334517 with a small business size standard of 1,000 employees. The VA Southern Nevada Healthcare System has a requirement for Hologic SmartCurve Breast Stabilization System. This is a solicitation open to Small Business only. All vendors must be properly registered in the System Award Management database (SAM) and Vetbiz.gov. This solicitation allows for vendors to offer BRAND NAME ONLY products. All items shall be new, and not refurbished or recycled. All deliveries must be completed by 30 Days ARO to the following destination: 5406 E. El Campo Grande Ave. Las Vegas NV 89115. Price quotes shall include the Duns Number, VIP Verification (if applicable), Small Business Status, an authorized distributor letter on the manufacturer s letterhead, and a copy of a price list or any other supporting information for the price submitted. The vendor shall provide new items ONLY; no remanufactured or grey market items. All items must be covered by the manufacturer s warranty. The offeror shall provide evidence of its authority to distribute the proposed equipment/system at the time of submission of its quote/offer. Please note that such evidence is subject to verification by the Contracting Officer at his/her discretion.
