This is a Sources Sought Constitutes Market Research The Department of San Francisco Veterans Affairs Medical Center (SFVAMC), CA is conducting market research to determine if there are sufficient number of qualified vendors to issue a Veteran-owned set aside or small business set aside. In accordance with United States Code Title 38 Section 8127(d), we are seeking vendors in the following categories: Service-Disabled Veteran Owned Small Business Veteran Owned Small Business Small Business The responses from this notice will be used to make the appropriate set aside determination. The proposed solicitation will be issued as a Request for Quotation under FAR Parts 12 and 13. The contractor shall provide pest control service for the SFVAMC. The Performance Work Statement is attached to this notice. Interested contractors should provide a capabilities statement demonstrating their experience providing similar services. The period of performance for this contract will be a base year plus four option years. All interested contractors must respond by email to Maria.Teodoro-Tanksley@va.gov before 1:00 P.M. Performance Work Statement Pest Control services San Francisco Veterans Affairs Medical Center General Information- 1.1- This is a non-personal services contract to provide fully certified and California licensed pest control services for the San Francisco VA Medical Center, Environmental Management Service, located at 4150 Clement Street, San Francisco, California 94121. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn, is responsible to the Government. 1.2- The Contractor shall furnish all labor, equipment, supplies, parts, supervision, management and transportation necessary to perform Integrated Pest Management Control services (IPM) (except mechanical or physical structure changes) in accordance with specifications, terms and conditions of this contract. 1.3-Point of Contact (POC): Chief, Environmental Management Service or his/her designee, #415-750- 6910. The Contracting Officer (CO) shall delegate the Contracting Officer Representative (COR) to monitor the contractor's performance. The delegation letter shall specify the limits of the COR's authority. 2. Required Contractor Reporting: Contractor shall sign in at the beginning of the treatment period and sign out at the end of the treatment period unless otherwise indicated by the POC or COR. The Contractor shall report to the Environmental Management Service building 2, Room 164 and provide a tentative itinerary for areas that shall be treated. 2.1 The following forms shall be filled out during the treatment and returned to the POC/COR prior to departing the VA facility: VA form 10-9020- Pest Management Maintenance Record VA form 10-9020a- Pest Management Operations Inspection Report VA form 10-9021- Pest Management summary Report (Quarterly) RCS 10-0134 (Quarterly report due by close of business on the last working day of October, January, April and July). 3. Estimated Visits: The Contractor shall report and log-in attendance with the COR/POC at three times a week, Monday, Wednesday and Friday on/about the hours of 7:00 am to 4:30 pm, to the EMS Office, Building 2, Room 164, tel# 415-750-6910. 3.1-Inspection and treatment of buildings/grounds to be made three times a week. Monday, Wednesday and Friday and as otherwise indicated in the specifications. Bed bugs treatment for 20 room with a minimum 2 bed per room. There is no limit to the maximum number of visits. However, visits must be made at a time satisfactory to the Officer in Charge of Pest Control. All buildings, parking structures and outside areas of the campus are covered under this statement of work. Several building will require treatment one or more times a month these building are Bldgs. 7, 5, 12, 200, 203 & 208. All buildings must be inspected and/or treated at a minimum of once a quarter. Upon departure, the Contractor shall check out In EMS Office, Bldg 2, room 164. 3.2- Call Back Requirement -If a treatment is inadequate (as per specifications), the Contractor shall return (within four (4) hours after receipt of notification by the COR) to re-treat previously treated area without cost to the government. 3.3.- Pest coverage includes Ants, Bees, Birds, Fleas, Flies and Mosquitos. We would also like to include the following types of ants Carpenter, Fire, Pharaoh and Raspberry Ants. 4-Areas of coverage are as follows: 4.1 Canteen Areas, ground and 1st floor, Bldg 7. The Canteen area may require treatment before or after normal working hours. This will be determined by the Contractor and Chief EMS and work will happen accordingly. 4.2 Dietetic Kitchen, Bldg. 203 basement. The Dietetic kitchen may require treatment after normal working hours. This will be determined by the Contractor and the Chief of EMS and work will happen accordingly. 5. Adding/Deleting visits and building/grounds The scope of this contract includes all the Medical Centers within the SFVAMC. If changes are needed by either adding visit or building within the SFVAMC it shall be covered under the scope of this contract. Any changes on the above buildings/grounds shall be handled through a bilateral contract modification. 6 -Treatment Services: Treatment shall be provided to control the population of ants, flies, bats, birds, roaches, moths, wasps, hornets, silverfish, spiders, mites, fleas, bedbugs, lice, mice, rodents and another related pest. (This excludes termites and mosquitoes). 7- Residual Treatment: In addition to the regularly scheduled visits, at least once every calendar month, the Contractor shall apply to all troubled areas a residual treatment for control of roaches. The contractor may use any individual formulations to a combination of formulations, provided they are compatible, approved and registered pesticides by the Environmental Protection Agency {EPA). Approved products of the residual type include Baygon, Diazinon and Durisban. Pyretrum is approved for the Non-Residual type. 8 -Restrictions on Pest Control Materials: All rodenticides, traps or bird repellents shall be placed only at times and in areas approved by the COR/POC and unused portions shall be recovered at the latest by a time limit set by the COR/POC designee. NOTE: No pest control material shall be placed where they may be recovered by patients, particularly in the Neuropsychiatric Wards. No pest control materials shall be allowed to meet food or cooking utensils. The use of rodenticides other than Red Squill, Warfarin and Pival, shall be restricted to locations specifically provided for by the Chief, EMS. The Contractor shall be required to provide the COR/POC initial application of pesticides and rodent control chemicals, sample labels, names of chemicals to be used in the performance of this contract. This information is required by the Medical Service for emergency treatment in the event of ingestion of and/or contact with the material by humans. A list of products and antidotes, Material Safety Data Sheets (MSDS) supplied/required by the Contractor and as required by this paragraph, should be maintained in the Pharmacy and in the Evaluation and Admissions Unit, where an antidote, if required, can be made readily available. 9. -Removal of Dead Rodents: All trapped rodents shall be disposed by the Contractor within a reasonable time frame as established by the Chief of EMS. 10. -Disposal of Hazardous Waste: The Contractor shall not dispose any excess pesticides, pesticide container or any material contaminated by pesticides at any location on the Medical Center premises except as specified by the COR/POC. 11. -Regulations 11.1 Pest management operations shall meet all Federal requirements as identified in Public Law 92-516, Federal Insecticide, Fungicide and Rodenticide Act, dated November 28, 1975, as amended, and Executive Order 12088 (Federal Compliance with Pollution Control Standards) and appropriate Occupational Safety and Health standards regarding hazard communication, personal protective equipment, and exposure standards. The Contractor shall ensure pesticides and pest control operations are in compliance with California Code of Regulations (Title 3, Food and Agriculture), Division 6, Pesticides and Pest Control Operation. Contractor shall conform with the State and local regulations governing, examining and licensing of pest control operators, in the use of pest control chemicals and equipment. 12. Authority of Applicator(s): Certified applicators are required in the performance of the work under the contract. Each applicator designated by the Contractor shall furnish a copy of his/her State Certification (showing categories) to the COR prior to reporting for work. All Applicators must have visible on their uniform, their name and company's logo. When there is more than one Contractor reporting, one shall be designated as a "Supervisor" and be fully certified in Pest Management categories. 13- Safety Requirements: The Contractor shall take precautions as the COR/POC may determine to be reasonably necessary to protect the lives and health of occupants at this Medical Center. The COR/POC shall notify the Contracting Officer of any non-compliance with the terms of the contract. Upon receipt of a Letter of Concern, the Contractor shall correct deficiencies and provide a corrective action letter to the CO. 13.1-Additional Precautions: The Contractor shall be responsible to ensure that employees take all necessary safety precautions appropriately in the performance of services i.e. wearing proper face mask, breathing apparatus, clothing, etc. The Contractor is also responsible to insure employees receive up-to-date training concerning the latest EPA, Federal, State and local regulations. 14.- Unscheduled Contingency Work: Unscheduled Contingency Work shall be required approximately 6 times per year. The Contracting Officer s Representative (COR) shall request this category of services based on urgency. In case of an emergency, the contractor is required to provide pest control service within four (4) hours after contingency work is requested. 15.- Quality Assurance Surveillance Plan 15.1 The Government shall monitor the contractor's performance using the Contractor s Basic Quality Control Program and Quality Assurance Surveillance Plan (QASP) as established by the COR. See Attachment #1 Quality Assurance Surveillance Program. 15.2 The QASP is a systematic method used by the Government to evaluate Contractor s actions and performance based on Performance Work Statement requirements. The QASP is designed to provide an effective method of monitoring contractor performance for each listed objective on the PWS. A QASP is based on the premise the Government desires to maintain a quality standard in providing pest control services. 16. - Contractor Contingency Plan: The Contractor shall have a contingency/back-up plan in the event the Contractor is unable to perform services or have services performed as required. The Contractor shall immediately notify the COR or designee and provide a justification for non-performance and institute the contingency and/or back-up plan as quickly as possible.
