The VA Palo Alto Health Care System s new community-based outpatient clinic (CBOC) to be located in San Jose, CA intends to issue RFQ 36C26119Q0606 to procure an ophthalmic ultrasound. This firm-fixed price solicitation is being conducted in accordance with FAR Part 13 Simplified Acquisition Procedures, and the corresponding NAICS Code is 334510. Please see RFQ document 36C26119Q0606 for requirements. Offerors are to follow the instructions in RFQ document 36C26119Q0606. An itemized quote as well as sufficient technical supporting documentation is mandatory. An authorized distributor letter will also be a requirement as a part of the solicitation package. This supporting documentation will be used to determine whether or not the quoted item(s) meet the requirements of the solicitation.
