This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures in FAR Part 13. This announcement constitutes the only solicitation. The solicitation number, 36C26119Q0274 is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-01 effective December 20, 2019 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to January 7, 2010. Interested persons may identify their interest and capability to respond to the requirement or submit a price quotation. The North American Industry Classification System (NAICS) Code for this acquisition is 334516, size standard 1000. This solicitation will be set aside for SDVOSB/VOSB if there are two or more Service-Disabled Veteran-Owned Small Business (SDVOSB) and Veteran-Owned Small Business (VOSB) who are capable of providing a quote at a fair and reasonable price. Unverified SDVOBs or VOSBs are considered non-responsive and price quotations will not be evaluated. Please provide price quotation using the attached RFQ. All price quotes received prior to 03/19/19 will be considered by the Government. Only emailed price quotations received directly from interested persons are acceptable. THIS IS A SOURCES SOUGHT NOTICE (a) The Government does not intend to award a contract on the basis of this sources sought notice or to otherwise pay for the information solicited. (b) Although proposal, offeror, contractor, and offeror may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this sources sought notice may be used in creating a solicitation. Any information received which is marked with a statement, such as proprietary or confidential, intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This sources sought notice is issued for purpose of collecting information about the availability of authorized distributors to provide prepared single unit doses of radiopharmaceuticals and associated materials to Martinez Nuclear Medicine at VANCHCS. Low Enriched Uranium Technetium shall be provided when at all possible, as mandated by VA Central Office. Orders shall be placed each day (Monday through Friday by 4:30PM) for delivery the next business day (Monday through Friday). Due to the demands of the Medical Center patient care requirements, contractor shall provide 3 routine deliveries at no charge. For additional orders placed during for the same day delivery, deliveries must arrive within 120 minutes of order placement. STAT order for the same day delivery must arrive within 60 minutes of order placement. Vendor must be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radiopharmaceuticals. This is a sources sought notice only, no formal solicitation document exists at this time. Offerors response shall include the following information: - Reference Number 36C26119Q0274 - Provide copies of licenses and certifications by the Nuclear Regulatory Commission - Company Name, point of contact, title, phone number and email address - DUNS number - Type of business (Small or large) and socioeconomic status whether Service Disabled Veteran Owned, Veteran Owned, Hubzone, 8(a), Woman owned, Small Disadvantaged, etc. or VOSB company to provide supplies for blood culture analysis. - State if your company has any government contracts against which this solicitation could be ordered (e.g., GSA, SEWP, NAC, GSA BPA, etc.) or if your proposal is submitted as a open market quote. For any government contract, provide your contract number, terms and conditions; SIN the products fall under (if any), delivery schedule and contract expiration date. - Contractors shall identify the NAICS code for the product being offered as well as their size status.
