DESCRIPTION OF REQUIREMENT Introduction: The VA Palo Alto Healthcare System (VAPAHCS) requires a contract vehicle to order sterile compounding pharmaceuticals in support of the Anesthesia Service. Background: Currently the Anesthesia Service in VAPAHCS uses approximately 100 On-Q pumps per month for post-op anesthesia. The critical care services use approximately 100 cases of Phenylephrine, 100 cases of Ephedrine and 15 cases of Epinephrine and 10 cases of Hydromorphone per month. The On-Q pump must be prefilled and primed in On-Q I Flow pump model CB0006 with On Demand and Select a Flow Rate control. The prefilled syringes of Hydromorphone, Phenylephrine, Ephedrine and Epinephrine with approved doses/volume are utilized to minimize dosing and medication errors. Minimum Requirements: The vendor must be state licensed and FDA registered 503B outsourcing facility. Pharmaceutical products are properly labeled to include but not limited to name of the medication, strength, volume/size, expiration date, lot numbers and any auxiliary warning labels if applicable. Products being ordered must be received within 2 business days of ordering. Any supply shortage issue from the vendor that results in failure to fill the order shall be communicated to the pharmacy procurement staff at the PAD pharmacy as soon as possible. The Sterile Compounding Pharmaceutical products being ordered and shipped to VAPAHCS will be carefully inspected. The pharmacy procurement staff will contact the vendor and send back any damaged product per inspection upon arrival for exchange.
