Description This requirement is for an open market competitive solicitation for community nursing home care services in the Southwest Washington geographical area. This is an open and continuous solicitation that is posted continuously for one year or longer, whereby nursing homes can apply freely in response to the list of current nursing home needs. The Government will periodically review proposals throughout that time and fill needs as required. This procurement concept is similar to the Government's Human Resources open and continuous job postings whereby a job is posted for an extended period of time and applications are reviewed periodically as needs arise. An RFP with the anticipation of up to 15 firm-fixed single award IDIQs with a max capacity of $3.0 M each, for a total RFP value of that which will not exceed $45 M. The proposed IDIQs will utilize Economic Price Adjustment clauses which are VAAR (852.216-71) Economic Price Adjustment of Contract Price(s) Based on a Price Index or VAAR (852.216-73) Economic Price Adjustment – State Nursing Home Care for Veterans. The VAAR clause selected for each IDIQ will be dependent on if the IDIQ uses Medicare or Medicaid. Nursing home facilities must be capable of furnishing the total medical, nursing, and psychosocial needs of VA beneficiaries. Rate adjustments will be based on the annual adjustment to the current published Medicare or Medicaid Benchmark Rate. Each nursing home facility must have a current Center of Medicare and Medicaid Services (CMS) certification (Medicare and/or Medicaid) and a state nursing home license. The VA requires CNHs to have bed capacity to ensure their ability to take referrals when requested. The CNH is responsible for providing care for VA beneficiaries including room, meals, nursing care, and other services or supplies commensurate with the VA-authorized level of care.

THIS IS A PRE-SOLICITATION NOTICE Solicitation Number: The proposed solicitation will be issued on an unrestricted basis all businesses are invited to participate. The NAICS code is 623110, Nursing Care Facilities (Skilled Nursing Facilities) with a $34M size standard. The solicitation will be issued as an open and continuous basis and will be available to vendors from the date of issuance of the solicitation and continuing for one year. During that period all interested parties are invited to submit a proposal for consideration. It is anticipated that the solicitation will be issued on/about April 25, 2024. This acquisition is to provide Community Nursing Home (CNH) services in the Southwest Washington catchment area. The following cities are included in this catchment area: Aberdeen, Camas, Centralia, Chehalis, Hoquiam, Kelso, Lacey, Longview, Olympia, Raymond, Shelton, South Bend, Tumwater, Vancouver, Washougal, Woodland, Yelm, Ridgefield The Contractor shall furnish all supplies and services, at the per diem rates for the Levels of Care specified.  Nursing home facilities in the CNH program shall ensure that care meets the health needs and promotes the maximum well-being of VA patients. Nursing home care will be furnished to ensure the total medical, nursing, and psychosocial needs of VA beneficiaries. All nursing home facilities in VA s CNH program must have current Center for Medicare and Medicaid Services (CMS) certification (Medicare and/or Medicaid) and a state nursing home license. VA often has a particular need for specialty care services in the CNH program. The VA requires CNHs to have bed capacity to ensure their ability to take referrals when requested. The CNH also must be able to accept VA referrals in a timely fashion (ideally within 24 hours of request). Provider visits will be available at the rate of one (1) visit per month. Laboratory, x-ray, and other special services will be available to VA patients as needed. In addition, the care provided will include room, meals, nursing care, and other services or supplies commensurate with the VA-authorized level of care, without extra charge. Duly authorized representatives of VA will provide quality oversight visits to veterans placed to assure continuity of care and to assist in the veterans transition back into the community, when applicable.  It is anticipated that up to 15 single award Indefinite Delivery Indefinite Quantity (IDIQ) contracts will be awarded from this solicitation. The capacity of each IDIQ will be based on the estimated value for a Nursing Home in each specific city and will range from $750,000 to $3,000,000. The period of performance for each IDIQ will be based on the award date and will continue for 5 years.
