This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of qualified sources to perform the potential requirement. This source sought is UNRESTRICTED and seeks providers capable of fulfilling the requirement in accordance with the Statement of Work. The responses received from interested contractors will assist the Government in determining set-aside and the appropriate acquisition method. The Department of Veterans Affairs (VA), Network Contracting Office (NCO) 20, is conducting market research to identify potential contractor sources which can provide the following services: Arjo Patient Mobility Lift Annual Preventative Maintenance Services for the Puget Sound VA Health Care System in accordance with the attached Statement of Work. Potential sources having the capabilities necessary to provide the above services which meet all the requirements outlined in the attached statement of work are invited to respond to this Sources Sought Notice via e-mail to Dane.Gift@VA.Gov no later than noted in the RFI. No telephone inquiries will be accepted. Responses should include the following information: Company name, address, and business size; point of contact name, phone number, and e-mail address; A capability statement addressing the requirements in the attached Statement of Work to include: References for similar services currently being provided or previously provided at the required service levels as outlined in the Statement of Work Capability Statement Capability to provide required services at the Puget Sound VA Health Care System in accordance with the Statement of Work NAICS Code 811219 is applicable to determine business size standard. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. The Government may or may not issue a solicitation as a result of this announcement. There is no solicitation available at this time. 2. Contract Title. Puget Sound Health Care System (PSHCS) Patient Ceiling Lift PM Contract 3. Background. Puget Sound Health Care System (PSHCS) requires an on-site PM inspection and repair contract to ensure that the patient ceiling lifts are kept in proper operating condition and ready to assist in lifting/moving its patients safely and effectively. This contract requires on-site preventative maintenance inspections, to include labor, batteries and straps as required, and completion of the Corrective and Preventive Maintenance Checklist for Ceiling Mounted Patient Lifts, ensuring that all units are fully operational according to manufacturer specifications. 4. Scope. Contractor shall provide all necessary travel, labor, materials, tools, and documentation required to provide load test, system check, and two page VA PM checklist completion for 362 ceiling mounted patient lifts. Lift batteries not changed within the past 18 months (as indicated on individual lifts) shall have new batteries provided. Labor for PM includes change of batteries, strap/belt if necessary, red emergency pull cord, and minor parts during performance of scheduled annual maintenance at the Seattle, American Lake, Mt Vernon, Port Angeles, and Silverdale locations. PM services shall be provided between 8:00 am to 5:00 pm, Monday through Friday, excluding federal holidays. 5. Specific Tasks. The contractor shall provide all travel, labor, materials, tools, and documentation for the annual on-site preventative maintenance inspections to ensure that the VAPSHCS ceiling mounted patient lifts (Appendix A) are fully in compliance with the Corrective and Preventive Maintenance Checklist for Ceiling Mounted Patient Lifts (Appendix B). This includes replacing batteries that are at least 18 months in service (as indicated by a dated battery service date sticker), and replacing emergency pull cords and lifting straps that fail to meet the manufacturer s criteria for PM compliance. 5.1 Task One: The contractor shall provide Annual Preventative Maintenance on the Seattle Division, American Lake, Mount Vernon, Port Angeles, and Silverdale ceiling mounted patient lifts. 5.1.1 Deliverable One: Annual Preventative Maintenance PM Services shall be in accordance with the Corrective and Preventive Maintenance Checklist for Ceiling Mounted Patient Lifts (Appendix B). 1. The contractor shall clean the system, 2. The contractor shall perform a system validation to include load test. 3. The contractor shall use replacement battery 12V 5AH SLA 187 FASTON batteries. Batteries that have been in service 18 months or longer, as indicated by a dated replacement label or annotated on the battery shall be replaced as above. 4. The contractor shall replace lifting straps and emergency pull cords that fail to meet the manufacturer s criteria for PM compliance 5. The contractor shall furnish a completed Corrective and Preventive Maintenance Checklist for Ceiling Mounted Patient Lifts (Appendix B) indicating the result of the scheduled inspection 6. Performance Monitoring 6.1. The contractor shall provide the COR a completed Corrective and Preventive Maintenance Checklist for Ceiling Mounted Patient Lifts for all work performed at the end of each day services have been performed. 6.2. The Corrective and Preventive Maintenance Checklist for Ceiling Mounted Patient Lifts shall identify any problems that arose, along with a statement explaining how the problem was resolved, in the appropriate comments block. This report shall also identify any problems that have arisen but have not been completely resolved, with an explanation. 6.3. Battery check list shall be completed for each battery change. 6.4. The tasks are well defined and can, therefore, be easily monitored by the Government POC, who shall represent the Government for this process. Contractor performance will be monitored and certified by the COR. The COR will assure contractor quality by routine inspections, product sampling, and monitoring of work while work is being performed. 7. Security Requirements The contractor employees shall not have access to VA sensitive or computer information and will not require routine access to VA Facilities. The contractor employees shall intermittent access only and will be escorted by VA employees while at VA Facilities. No background investigation is required. Records Management Language for Contracts The following standard items relate to records generated in executing the contract and should be included in a typical Electronic Information Systems (EIS) procurement contract: Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). Government furnished property will not be provided to the contractor. 9. Other Pertinent Information or Special Considerations. 9.1. Identification of Non-Disclosure Requirements. It is agreed that: 9.1.1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. 9.1.2. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. 9.1.3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. 9.2. CONTRACTOR EXPERIENCE REQUIREMENTS 9.2.1. Each respondent shall have an established business, with an office and full time staff. The staff includes a "fully qualified" Field Service Engineer (FSE) and a "fully qualified" FSE who will serve as the backup. "Fully Qualified" is based upon training and on experience in the field. For training, the FSE(s) has successfully completed a formalized manufacturer training programs within the last two years, for the equipment identified in the equipment schedule, and annual refresher course. For field experience, the FSE(s) has a minimum of two years of experience, with respect to scheduled and unscheduled preventive and remedial maintenance, on equipment listed. The FSEs shall be authorized by the contractor to perform the maintenance services. All work shall be performed by "Fully Qualified" competent FSEs. The contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FSEs for each make and model the contractor services at the PSHCS. The CO may authenticate the training requirements, request training certificates or credentials from the contractor at any time for any personnel who are servicing or installing any PSHCS equipment. The CO and/or the COR specifically reserve the right to reject any of the contractor's personnel and refuse them permission to work on the PSHCS equipment. Subcontractor will not be used under this contract unless authorized and approved by the COR. CHANGES TO STATEMENT OF WORK 9.3.1 Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. 10. Risk Control The equipment covered under this service contract is required to be fully operational according to manufacturer specifications, and any discrepancies need to be brought to the attention of the facility so appropriate repairs can be implemented. The equipment ensures the safety of patients and staff. 11. Place of Performance. Period of Performance. The contractor shall complete the work required under this SOW in once given the award for and then for (1) one year unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail.
