(i) This is a combined synopsis/solicitation for Radiological Physics Testing & Services for the Portland VA Health Care System and surrounding CBOC s, as prepared in accordance with the format in Subpart 12.6, as supplemented in the attached Request for Quotes (RFQ). A firm-fixed price purchase order is anticipated. Simplified Acquisition Procedures IAW FAR 13 will be used. (ii) The solicitation number is 36C26020Q0391 and is attached. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-05 effective 03/30/2020. (iv) This solicitation will be set-aside small businesses. Qualified offers will be evaluated using comparative evaluation process. The associated NAICS 621512 code has a small business size standard of $15M. The person performing the work must be an American Board of Radiology certified Medical Physicist. The vendor shall provide new items ONLY; no remanufactured or "Gray Market" items. All items must be covered by the manufacturer's warranty. (v) Quotes must be emailed to Amy.Kuczajda@va.gov and received no later than 12:00pm MST on 05/05/2020. Quotes may be submitted on the attached RFQ. No late quotations will be accepted.
