Page 2 of 3 REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY The Department of Veterans Affairs Network Contracting Office (NCO) 19, is seeking information and sources that are capable of providing certified Extracorporeal Shock Wave Lithotripsy (EWSL) Services to beneficiaries of the Department of Veterans Affairs (VA) and the VA Salt Lake City Healthcare System (VASLCHS). Scope of work: The Contractor s technical personnel shall be certified and trained in ESWL services and operations of the unit(s). The Technician will assist the attending Urologist in the operation of the device. The technician may be required to operate the device and any related imaging equipment during procedures. Prospect contractor s technicians(s) shall have a current license to perform services in the state of Utah when services are performed. Place of Performance: Place of Performance: Contractor shall provide on-site services at VASLCHS 50.0 Foothill Blvd. Salt Lake City, Utah, 84148. If this request for information leads to a future solicitation, the Government intends to award a firm-fixed price contract. The performance period shall be one (1) base year with four (4); one (1) option years. This is a request for information and sources only, which may or may not lead to a future solicitation! This is not a request for proposal (RFP). The VA will not pay for any information received resulting from this sources sought notice. Requests for copies of a solicitation shall not be honored or acknowledged. Information should be forwarded to the Contract Specialist. Because this is a request for information only, answers to questions will not be posted.
