Box 1500 Fort Harrison, MT 59636-1500 1.877.468.8387 | 406.442.6410 www.montana.va.gov Statement of Work Renovate Surgery Out-Patient Clinic Scope: The VA Montana Healthcare System requires the services of a General Contractor for the renovation of six rooms on Level 3 of Building 154 as indicated in the drawings. Americans with Disabilities Act (ADAAG Standards), Architectural Barriers Act Accessibility Standard (ABAAS) and the VA Barrier Free Design Standards will be implemented throughout the renovation. Renovation work is to include but not limited to all new interior finishes, wall protection, replacement doors and hardware, mechanical and electrical system upgrades, fire sprinkler, fire alarm, building integrated medical equipment, nurse call, security and medical gasses. The Contractor shall also be responsible for all damages to persons or property that occurs as a result of the Contractor s fault or negligence. The Contractor shall also be responsible for all materials delivered and work performed until completion and acceptance of the entire work, except for any completed unit of work which may have been accepted under the contract. Existing Site Conditions. Construction will occur on Level 3 of Building 154 just West of the main hospital elevator. ICRA Requirements: The project will take place in an occupied healthcare facility and the construction activities will require construction to be phased in such a way as to minimize impact to staff and patients. Infection Control measures will be required at all times as defined in ICRA assessment, Class III, Group 2, Type C. Construction activity will require constant coordination with VA personnel. Work shall be completed in conformance with the time schedule requested by the VA Contracting Officer s Representative (COR). The Contractor is solely responsible for preparing all necessary regulatory notifications to meet the time schedule. Site Investigation is required for this project to ensure accuracy of design. The Contractor shall be responsible for field verifying all conditions and quantities that may affect the project work prior to bid of this project. The final construction shall conform to all VA requirements outlined in the FGI Guidelines for Design and Construction of Hospitals, Facilities Guideline Institute (FGI), ADA, Joint Commission Accreditation Standards, ANSI, SMACNA, IBC, NEC, NFPA as well all requirements of the VA design manuals posted by discipline in the VA Technical Information Library at the following web address: http://www.cfm.va.gov/til/dManual.asp Due to requirements in NFPA 101 Life Safety Code, a minimum of two egress routes for each occupied space must be maintained after an allowable common path of travel distance of 100 feet (per NFPA 101 Paragraph 18.2.5.3. To ensure the safety of VA patients, staff, and tenants, work is to be structured to allow for egress routes to be maintained throughout the course of construction. Work requiring relocation of tenants, is subject to notification requirements. If Contractor provides a schedule resulting in any violations of NFPA 101 Life Safety Code during any work, they must also submit proposed interim life safety measures for VA review. Any action necessitated by NFPA 101 Life Safety Code will be the responsibility of the Contractor. Background and Facility: Existing Site Conditions, the project will take place in an occupied healthcare facility and the construction activities will require construction to be phased in such a way as to minimize impact to staff and patients. Construction activity will require coordination with VA Contracting Officers Representative (COR) and designated VA personnel. All demolition shall be required to remove abandoned utilities back to active main (e.g., medical gases, water); site investigative efforts are to facilitate that component of the work. Construction Requirements: Construction of the above documented project scope must meet all applicable codes, regulations, and standards. This includes but is not limited to NFPA Codes, NEC, EPA (specifically conduction of NEPA), MT DEQ Regulations, OSHA 1926, ASTM and TJC Standards. Additionally, the General Contractor and its Sub-Contractors are required to meet all applicable laws and regulation not explicitly stated in this document. All efforts and products shall be conducted and produced in compliance with VA Publications including Master Construction Specifications, Design Guides, and CAD Standards (http://www.cfm.va.gov/TIL). Submittals issued during construction will be limited to all MEP components. Associated architectural submittals will be agreed upon prior to commencing construction. Upon completion of construction, contractor will produce As-Built drawings to the VA. As-Built drawings to be provided electronically in both AutoCAD and PDF. These drawings are to be stamped as AS-BUILT . Operational and Maintenance manuals provided are to include a single hard copy and a complete three electronic copies. Projects are to be assessed for impact to Patient Care utilizing an Infection Control Risk Assessment (ICRA) in advance. Where the ICRA is found to represent significant impact to the means and methods of construction of individual tasks, the requirements of the ICRA will be included on a drawing sheet for each tasking. Additional impacts identified by the VA may be required to be included in the drawing sets to address other impacts to construction as a means of ensuring continuation of operations is maximized throughout the construction period. 4. Schedule All work efforts as related to construction are to be completed in duration not to exceed 90 calendar days (i.e., see Period of Performance section for full details). 5. As-Builts Drawings Following Construction, the General Contractor and its Sub-Contractors shall complete as-built drawings and furnish them electronically in PDF format on a removable storage format to the VA. 6. Post-Construction Quality Validation Within the specification, the General Contractor is to specify a one-year warranty period covering defects related to all work and materials/equipment included in the project. The General Contractor is to review the project in its entirety and determine oversight or resource requirements to enhance sustainment and limit need for warranty period services. Factory start-up is required of all applicable equipment included in the project. Where failures during the warranty period are attributable to an element of the design, the General Contractor and their sub-contractors are to provide consultation and re-design services at no cost to allow for remediation. Mandatory Tasks: Required Submittals Construction and Phasing Schedule Proposed Utility Shut-off Schedule Mechanical equipment, mechanical equipment layout, ductwork and ducting layouts and diffusers. Electrical power, Low Voltage and Lighting layouts. Plumbing Fixtures and associated appurtenances. Fire suppression and installation layouts. Fire alarm and smoke detection device and installation layout. Roofing materials, roof installation details, roof penetration and curb details and warranty information. Interior and wall protection finishes. Submittals must be reviewed and approved by the VA prior to the start of work. All demolition shall be required to remove abandoned utilities back to active main. All selective demolition will be performed at the time of each proposed phase of construction. Any additional selective demolition outside of the current phase of work will need to be approved by the COR. All selective demolition is to occur after normal working hours and limited to a single work shift. Contractor shall be responsible for all construction debris removal. The contractor shall provide their own dumpster as needed. Dump carts are to be sealed with a tight-fitting lid during travel to and from the jobsite to the designated dumpster location. Dumpster location and debris removal pathway to be approved by the VA COR. Work area and debris removal pathways from the jobsite to the dumpster to be kept vacuum clean at all times. All damage to floors, walls, ceilings, or any other VA property to be repaired/replaced by the contractor. Installation shall minimize penetrations in finished walls to the extent possible and with any such penetrations repaired to match existing construction or meet and/or exceed required sound transmission coefficient (STC), smoke/fire partition rating requirements after completion of work. Any new construction shall match surrounding finishes unless indicated otherwise. Any new electrical power and lighting work shall tie into closest available circuit or panel. Contractor and their Sub-contractors are to provide necessary submittals and plans for approval prior to commencing with any new work not shown/provided in the construction documents. The intent is for existing mechanical systems to be reused as much as possible without significant modification or alteration to the existing system. Temporary egress and life safety signage to be provided by contractor and any ILSMs required shall be coordinated with VA COR and VA safety personnel. Fire sprinkler protection must be maintained at all times. Sprinkler heads located within ceiling tiles must be re-installed at the end of every work shift. Contractor shall coordinate the removal of existing ceilings with the installation of new ceiling grid and new ceiling tiles. If not, the installation of temporary upright sprinklers will be required. After work is complete, new pendent sprinklers must be installed as sprinklers which are removed cannot be reinstalled. To maintain the use of non-rated construction barriers, fire sprinkler coverage must be maintained at all times. If for any reason planned sprinkler outages cannot be limited to a single work shift, the installation of fire rated barriers will be required to separate the affected construction area. Temporary barriers, as indicated on the proposed Phasing Plan, are just that, temporary. Temporary barriers are to be considered non-rated construction barriers. Temporary barriers are limited to a single work shifts only. Temporary barriers will be erected at the beginning of the work shift and dismantled in their entirety at the end of the work shift. Temporary barriers should seal tightly at walls and ceilings. Entry doors should have either magnetic or zipper seal. Temporary barriers should be able to withstand the use of negative air pressure within the work zone/containment area. Use of pre-manufactured, re-usable barriers panels is acceptable. Use of fire-retardant sheet plastic barriers is acceptable as long as the plastic barrier is free from punctures, rips or tears. Use of tape or similar patches will not be allowed. The entire plastic barrier is to be replaced at the contractor s expense. See Section 01 35 26 Safety Requirements for additional information and clarity. Hard wall barriers, as indicated on the proposed Phasing Plan are to be interior partitions constructed to a minimum 1 Hour rating throughout the duration of the proposed phase. Hard wall barrier to meet/exceed UL U419 assembly, interior partition Steel stud, Non-Load bearing. See Section 01 35 26 Safety Requirements for additional information and clarity. At no time will Hard wall barriers be allowed to span a corridor unless indicated on the Phasing Plan and/or approved by the COR during construction. Description of Work: Level 3 of Building 154 Selective Demolition (as indicated in drawings) - Procedure Room 373: vinyl composite tile flooring, headwall units and associated medical gasses, power devices, data devices, nurse call devices, miscellaneous low voltage devices, recessed light fixtures, abandoned junction boxes, mechanical radiator units, handwash sink and faucet set, tile finishes, ACP ceiling system, ceiling light fixtures and existing linear diffusers. Selective Demolition (as indicated in drawings) Rooms 344, 345, 346 371 and 372: vinyl composite tile flooring, headwall units and associated medical gasses, power devices, lighting control devices, data devices, nurse call devices, miscellaneous low voltage devices, recessed light fixtures, abandoned junction boxes, shower and tile, handwash sink and faucet set, tile finishes, ACP ceiling system, ceiling light fixtures and existing linear diffusers. Existing patient lift system to be removed by the General Contractor and returned to the VA COR or VA COR s designated representative. Procedure Room 373 (as indicated in drawings): Work to include but not limited to LVT flooring, flooring transitions, wall base, partitions, interior wall finishes, privacy curtains, window treatments, wall protection, pre-manufactured casework and countertops, acoustical ceiling tile and ceiling grid, light fixtures, lighting controls, mechanical diffusers, mechanical controls, electrical power devices, plumbing fixtures and faucet controls, fire suppression, fire alarm devices, nurse call devices. Coordinate associated MEP trades as needed to complete the work as needed for clinic occupancy. Rooms 344, 345, 346, 371 and 372 (as indicated in drawings): Work to include but not limited to LVT flooring, flooring transitions, wall base, partitions, interior wall finishes, privacy curtains, window treatments, wall protection, pre-manufactured casework and countertops, acoustical ceiling tile and ceiling grid, light fixtures, lighting controls, mechanical diffusers, mechanical controls, electrical power devices, plumbing and faucet controls, fire suppression, fire alarm devices, nurse call devices. Coordinate associated MEP trades as needed to complete the work as needed for clinic occupancy. Period of Performance: Upon award of this project, the Contractor is required to attend the pre-construction meeting where it will be determined whether an Administrative Notice to Proceed (ANTP) will be issued. During this time all required pre-construction submittals such as, safety plans, quality control plan, phasing, schedules, materials submittals, certifications, etc. shall be provided for VA review and approval. Additionally, the ANTP is when the Contractor will place orders for any required equipment and materials due to increases in lead times, fuel surcharges and material costs. Following satisfactory submission of all pre-construction submittals and the arrival of required equipment and materials, a Construction Notice to Proceed (CNTP) will be issued. Thus, the period of performance for this project is 120 days from the CNTP. Work Performed After Hours: Work is expected to be performed after hours to maintain construction schedule and to provide access to the hospital during normal working hours. All Work that is to occur after Normal working hours and limited to a single work shift. After Hours, Single work shift hours, Monday - Friday, are from 4:30 PM to 7:30 AM. If portions of the afterhours Work need to be extended to be completed during Normal working hours, work performed during Normal working hours will need to be approved and coordinated by the VA Contracting Officers Representative (COR). In order to maintain schedule, work performed over the weekend, particularly overhead and new ceiling work will be allowed. Contractor to coordinate these efforts with VA COR with a minimum 48-hour notice. Privacy Statement Information Security: The C&A requirements do not apply; a Security Accreditation Package is not required. Incidental exposure to protected health information may occur from accessing the work site. Staff will follow all Privacy & Security policies and procedures.
