Statement of Work Rocky Mountain Regional VA Medical Center Kitchen Equipment Maintenance and Repairs Services 1.0 Introduction: 1.1. The Department of Veterans Affairs at Rocky Mountain Regional VA Medical Center (RMRVAMC) currently has the need for a service contract to perform preventative maintenance and urgent repairs of commercial kitchen equipment located at 1700 N. Wheeling Street, Aurora, Colorado, 80045 The period of Performance shall be 1 Base year of 12 months and four (4) 12-month option periods. 2. Objectives: 2.1. Establish a contract with a reputable company to provide maintenance and repair services outlined in this SOW for kitchen equipment located at the medical facility in Aurora, Colorado. 3. Scope: 3.1. The contractor shall furnish all labor, material, equipment, parts, supervision, etc., necessary to maintain and repair equipment listed in paragraph 3.1.1. This equipment is located in the main kitchen, Spinal Cord Injury (SCI) kitchen, and Canteen kitchen at the Rocky Mountain Regional VA Medical Center. 3.1.1. Contractor shall perform preventative maintenance in accordance with original equipment manufacturers (OEM) standards on the specified equipment. Preventative maintenance will include: 3.2.1. Check, adjust, clean, and service equipment according to the manufacturer recommendations /specifications 3.2.2. The examination, lubrication, adjustment, and calibration of parts to include but not limited to belts, bearings, capacity, and safety devises, check valves, condensate pumps, coils, control transformers, control wiring, dampers, electrodes, all filters, fuses, environmental controls, fans, motors and motor starters, pressure controls, compressors, refrigerant filters drier, refrigerant piping relays and all necessary maintenance require to keep the units in proper and continuous operation. 3.2.3. Routine testing and inspecting equipment to reduce or avoid service interruptions and to obtain optimum operating efficiency and maximum life expectancy of equipment. 3.3. Preventative maintenance will be scheduled and coordinated with government point of contact (POC). 3.4. All labor, parts, and supplies for maintenance and repairs shall be paid for and provided by the contractor. 3.5. The contractor shall maintain records of all service visits, maintenance and repairs on equipment specified in this contract. Contractor shall provide service reports within five (5) business days of work being completed. 3.6. This contract covers an unlimited number of calls for repairs and maintenance. 4. Contractor s Qualifications: 4.1. Contractor shall have been in operation for at least five (5) years and have at least (5) years of commercial equipment repairs and service experience. 4.2. Shall have current licenses/certifications required for HVAC, plumbing and heating, refrigeration & cooling work. 4.3. Contractor who receives this contract shall have access to Hobart engineers for troubleshooting assistance/technical support and shall be certified to service. 4.4. Contractor shall be able to test and troubleshoot all kitchen equipment and all brands. 4.5. Contractor shall be able to reprogram equipment operated with digital timers. 4.6. Response Time Levels by Call type as provided: Emergency, upon receipt of call, contractor shall dispatch a repair technician to arrive on site within four (4) hours. Non-Emergency, upon receipt of call, contractor shall dispatch a repair technician to arrive on site within twenty-four (24) hours. General: 5.1. Working hours: Anticipated hours of work will be Monday through Friday from 7:30am 4:00pm. Emergencies will be on a case-by-case basis. Additional hours must be coordinated through the CO. Work will not be performed on weekends or federal holidays. Observed Federal Holidays: Columbus Day Veteran s Day Thanksgiving Day Christmas Day New Year s Day Martin Luther King Birthday Washington s Birthday Memorial Day Juneteenth Independence Day Labor Day 5.2. Safety: Contractor is responsible for maintaining a safe working environment for their employees as well as the RMRVAMC Staff. Work areas will be kept clean throughout the day and all trash and materials properly disposed of daily. Appropriate Personnel Protective Equipment will be worn while performing work on the VA Campus. 5.3. Security: Contract personnel need to obtain a vendor s badge form VA Security located at the outpatient entrance security desk. A valid government issued form of identification will be required to receive a badge. Contractor s Technicians are required to check in with the COR or Energy Center Supervisor upon arrival at facility to obtain access and any additional instructions, task explanations and project location information. Daily performance log sheets are required to be submitted to the CO and COR daily. 5.4. Parking/Storage: Parking is available on campus and will be coordinated with the contract POC following award.
