SYNOPSIS THIS IS NOT A SOLICITATION. THIS IS A SOURCES SOUGHT NOTICE ( SOURCES SOUGHT ). RESPONSES TO THIS SOURCES SOUGHT SYNOPSIS ARE NOT CONSIDERED ADEQUATE RESPONSES TO THE SOLICITATION ANNOUNCEMENT. ALL INTERESTED OFFERORS WILL HAVE TO RESPOND TO THE SOLICITATION ANNOUNCEMENT IN ADDITION TO RESPONDING TO THIS SOURCES SOUGHT ANNOUNCEMENT. Due to length of time, which has passed since a market survey was previously performed for Vernal Community Based Outpatient Clinic Services, this Sources Sought is posted to conduct additional market research IAW FAR Part 10. The Government is seeking information to determine the availability of potential sources having the skills and capabilities necessary to provide the appropriate level of care to the beneficiaries the Department of Veteran Health Administration (VHA). The Veterans Affairs Salt Lake City Health Care System has a requirement for a Community-Based Outpatient Clinic (CBOC) within the city limits of Vernal, UT. The award of a Fixed Price Indefinite Delivery Contract (IDC) is anticipated. The estimated period of performance is one (1) year base period followed by four (4) one-year options. Please be aware all Federal, State and local labor laws will be applicable to this procurement including the federal minimum wage as provided in Section 6(a)(1) of the Fair Labor Standards Act. The number of veterans requiring outpatient care will fluctuate during performance and cannot be confidently estimated for the entire contract, but it is currently estimated that 810 veterans will need care at the start of performance. Additionally, the VHA is seeking information in order to gain knowledge of any potential qualified sources and their size classifications, including Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub Zone, 8(a), Small, Small-Disadvantaged, Woman-Owned Small Business, FSS/GSA contract schedule holders or large business relative to NAICS 621498 All other Outpatient Care Centers (size standard $25.5 M). This Sources Sought is issued solely for informational planning purposes only and does not constitute a Request for Proposal (RFP); any promise to issue an RFP; or commit the Government to a contract for any supply or service. The VHA is not seeking proposals now and will not be responsible for costs incurred in response to this Sources Sought. All costs associated with responding to this Source Sought shall be at the interested respondent s expense. Not responding to this Sources Sought does not preclude participation in any future RFP, if any is issued. Any information submitted by respondents to this Sources Sought is strictly voluntary. All submissions shall become the property of the VA and shall not be returned. This announcement is based upon the information available at this time and is subject to future modification. REQUESTED INFORMATION: (1) STATEMENT OF CAPABILITY: Submit a brief description, five (5) pages or less, that demonstrates how your business can provide the requested services at time of award. Include your business past experience in performing these services to the VA, other Government (Federal or State) agency, or for a private facility. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small as defined by the NAICS (b) If small, indicate if your business qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your business is certified under Section 8(a) of the Small Business Act (d) Indicate if your business is a certified Hub-zone business (e) Indicate if your business is a woman-owned or operated business (f) Indicate if your business is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your business (h) In accordance with FAR 52.204-7, System for Award Management, prospective contractors must be registered in System for Award Management (SAM) and in the Small Business Administration VetCert database if applicable (VetCert certification is required for Service-Disabled Veteran Owned Small Businesses or Veteran Owned Small Businesses) under the applicable NAICS. Contractors may obtain SAM information at SAM.gov Home and Vet Cert information at Veteran Small Business Certification (sba.gov). To be eligible for award, businesses must be registered in the System for Award Management database. Registration is free and potential offerors are encouraged to visit the SAM website as no IDCs can be awarded to any offeror not registered in SAM.
