REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY The Department of Veterans Affairs Network Contracting Office (NCO) 19, is seeking information and sources that can provide The contractor shall provide Registered Nurse, Specialty Registered Nurse, Licensed Practical Nurse, Specialty Licensed Practical Nurse, Nurse Assistant, Medical Assistant, Surgical Technologist and Specialty Surgical Technologist Services for Acute Hospital Clinical Service, Extended Care, Mental Health and Community Living Center units on site in accordance with the terms and conditions contained herein to beneficiaries of the Department of Veterans Affairs (VA), Rocky Mountain Regional VA Medical Center (RMRVAMC). PLACE OF PERFORMANCE: The services performed under this contract may be performed at all RMRVAMC access points including Community Based Outpatient Clinics (CBOC) and Community Living Centers (CLC). The geographical coverage area identified in the Contractor s FSS Contract shall include Colorado. The list below includes existing locations but may be subject to change. EASTERN COLORADO HEALTH CARE SERVICES Rocky Mountain Reginal VA Medical Center Denver - Community Resource Referral Center Lakewood - Valor Point Domiciliary Aurora CBOC Jewel Clinic Golden CBOC La Junta - CBOC Colorado Springs CBOC Colorado Springs Surgery & Specialty Clinic Colorado Springs Union Boulevard Colorado Springs Union Blvd 1 Colorado Springs Space Center Castle Rock CBOC Pueblo - CBOC Pueblo - CLC Alamosa - CBOC Salida Patient Tele-Health Outpatient Clinic (PTOC) Lamar - PTOC Burlington PTOC The information submitted shall contain the company s business size status. This is a request for information and sources only, which may or may not lead to a future solicitation! This is not a request for proposal (RFP). The VA will not pay for any information received resulting from this source sought notice. Requests for copies of a solicitation shall not be honored or acknowledged.
