Willow Drive, Suite 300, Greenwood Village, CO 80111 intends to solicit sources to enter into a firm-fixed price, Blanket Purchase Agreement (BPA) to provide Microbiology Culture Media for the following VISN 19 locations: Rocky Mountain Regional VA Medical Center (RMRVAMC) located at 1700 North Wheeling Street, Aurora CO, 80045, VA Cheyenne Health Care System located at 2360 East Pershing Blvd., Cheyenne WY, 82001 VA Sheridan Health Care System located at 1898 Fort Road, Sheridan, WY 82801 VA Montana Health Care System located at 3717 Veteran s Drive Fort Harrison MT, 59636 VA Salt Lake City Health Care System located at 500 Foothill Dr., Salt Lake City, Utah 84148 VA Oklahoma City Health Care Systems located at 921 NE 13th St. Oklahoma City, OK 73104 VA Muskogee Health Care System located at 1011 Honor Heights Dr., Muskogee, OK, 74401 The contractor shall provide a full range/wide array of microbiology media, supplies, identification kits, biochemical reagents for rapid/full identification, American Type Culture Collection (ATCC) quality control (QC) organisms, and disposable supplies related to processing and complete work-up of specimens for microbiology cultures. Media, kits, reagents, and QC-organisms are specific for required microbiology testing for patient specimens, and quality control requirements.
