Page 2 of 2 REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY The Department of Veterans Affairs Network Contracting Office (NCO) 19, Rocky Mountain Acquisition Center (RMAC) is seeking information and sources that can provide Microbiology Culture Media and Supplies for Veterans Integrated Service Network (VISN) 19. Contractor shall provide a full range/wide array of microbiology media, supplies, identification kits, biochemical reagents for rapid/full identification, American Type Culture Collection (ATCC) quality control (QC) organisms, and disposable supplies related to processing and complete work-up of specimens for microbiology cultures. Media, kits, reagents, and QC-organisms are specific for required microbiology testing for patient specimens, and quality control requirements. Supplies shall shipped/delivered to Rocky Mountain Regional VA Medical Center (RMRVAMC) located at 1700 North Wheeling Street, Aurora CO, 80045, VA Cheyenne Health Care System located at 2360 East Pershing Blvd., Cheyenne WY, 82001, VA Sheridan Health Care System located at 1898 Fort Road, Sheridan, WY 82801, and VA Montana Health Care System located at 3717 Veteran s Drive Fort Harrison MT, 59636, VA Salt Lake City Health Care System located at 500 Foothill Dr., Salt Lake City, Utah 84148, VA Oklahoma City Health Care Systems located at 921 NE 13th St. Oklahoma City, OK 73104, and the VA Muskogee Health Care System located at 1011 Honor Heights Dr., Muskogee, OK, 74401. The NAICS code is 325413 In-Vitro Diagnostic Substance Manufacturing and Small Business Size Standards is 1,250 employees. The information submitted shall contain the company s business size status.
