PURPOSE OF THE NEW RFQ: The previous RFQ 36C25920Q0640 was cancelled in its entirety. The intention of posting this replacement RFQ 36C25921Q0077 is to alleviate confusion in the requirement based on the previous PWS and to provide a clearer description of the requirement in the revised PWS. The changes to the Performance Work Statement simply clarify the entire requirement. Amendment 0002 issues a cancelation to RFQ 36C25920Q0640 on November 10, 2020. Vendors must follow Addendum to FAR 52.212-1 Instructions to Offerors Commercial Items , located in Section E of the new RFQ. ALL QUOTES MUST REFERENCE THE NEW RFQ 36C25921Q0077. BACKGROUND: the same services were previously contracted on VA256-16-C-0053. SET ASIDE: This is set aside 100% for Service-Disabled Veteran Owned Small Businesses (SDVOSBs). THE REQUIREMENT: Oklahoma City VA Health Care System requires the contractor to complete preventive maintenance and repair services on phacoemulsification units: cataract extraction (OEM: Alcon Vision), in accordance with the PWS. Refer to the attached solicitation. This is a non-personal services contract. The Contractor, its employees, agents and subcontractors shall not be considered VA employees for any purpose of fulfilling the PWS and shall be considered employees of the Contractor. Contractor shall furnish all labor, materials, parts, equipment, tools, shipping, transportation, insurances, licenses, certifications and supervision necessary to provide services in accordance with this Performance Work Statement (PWS). The Contractor is responsible for all charges and fees related to disposal. For a contract of this size, scope and complexity, the successful Contractor must perform in a professional manner as recognized by commercial industry standards as good to excellent.
