Testing cards, diluent and supplies shall be on a BPA and ordered as needed. Contracted vendor shall provide products to be of a quality meeting or exceeding current recognized national standards as established by The Joint Commission and College of American Pathologists. Contractor shall provide reagent test kits and supply required to perform the following tests on the MTS gel equipment: 0.8% Affirmagen (2x10mL), 0.8% Surgiscreen (3x10mL), Confidence System (Kit), A/B/D Mono and Reverse Card (5x20), Anti IgG Card (5x20), Buffered Gel Card (5x20), A/B Mono Card (5x20), A/B/D Mono Cards (5x20), MTS Pipette Tips (12x96), MTS Diluent 2 (5x100mL), MTS Diluent 2 Plus (5x100mL), MTS Diluent Dispenser (0.5mL), MTS Diluent Dispenser (1.0mL) The NAICS Code: 325413- In-Vitro Diagnostic Substance Manufacturing, PSC: 6650 - In-Vitro Diagnostic Substances, Reagents, Test Kits and Sets. Small Business Size Standard: 1,250. *All responses should include the following; FSS contract number if applicable, socioeconomic category, technical/capability information, ability to meet VA requirements, and existing customers. Interested sources may submit a 1 or 2-page capability statement with enough detail for the Government to make an informed decision regarding such capability. The statement should identify supplier s name, address, point of contact, phone and fax numbers, e-mail addresses and business type and size. Any pertinent FSS Schedule numbers with expiration date should also be provided.
