The Department of Veterans Affairs has a requirement for a contractor to provide Preventative Maintenance Services and/or a Service Agreement in accordance with this brief Statement of Work (SOW). All work shall be coordinated with the Jack C. Montgomery VA Health Care System s COR. Location of Performance: Jack C. Montgomery VAMC 1011 Honor Heights Drive Muskogee OK 74401 GENERAL: The Jack C. Montgomery VA Medical Center is requesting a Preventative Maintenance Services and/or a Service Agreement for 5 (five) Xenex robot germ zapping machines. The Machines where purchased in January of 2018 and came with a 2-year warranty. The warranty has now passed, and we have a need to cover all repairs and parts to include bulb replacement and software upgrades (as needed or recommended by the OEM). PERIOD OF PERFORMANCE: Our goal is a Base Contract with 4 possible Option Years. SCHDULE OF SERVICES: Contractor shall provide all services to include bulb changes affiliated with the repairs and information reporting associated with this contract. Repairs will be scheduled in a timely manner as to not hinder the safety and well being of any patients, guest and employees at the facility.
