THIS IS NOT A REQUEST FOR PROPOSAL. The Havre Community Based Outpatient Clinic located in Havre, MT and the Network Contracting Office (NCO) 19 are performing market research to determine if there is sufficient number of qualified providers/firms for Reference Laboratory Services. Socio-economic precedence are as follows: (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small Business to set aside GENERAL REQUIREMENTS Firms responding shall specify they can meet the required specifications: Furnish all management, supervision, and labor necessary to perform collection and processing services for Havre Community Based Outpatient Clinic in support of veteran patients. Services required are: 1) Collection and processing of all venous blood samples and urine samples from veteran patients as ordered by VA clinician. 2) Proper labeling of the patient samples using the computerized labels supplied with the printed order. 3) Processing and packaging of those specimens according to provided directions and preparing a daily box for courier pickup (transport charges paid by VA).
