THIS IS NOT A REQUEST FOR PROPOSAL. The VA Salt Lake City Health Care System (VASLCHCS) and the Network Contracting Office (NCO) 19 are performing market research to determine if there is sufficient number of qualified providers/firms for courier services. Socio-economic precedence are as follows: (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small Business to set aside GENERAL REQUIREMENTS Firms responding shall specify they can meet the required specifications: The contractor shall provide all necessary labor, equipment and transportation services each week-day for pick-up and delivery between VA Salt Lake City Health Care System (VASLCHCS), the St. George VA Community Based Outpatient Clinic (CBOC), and the Pocatello VA Community Based Outpatient Clinic (CBOC). The contractor shall deliver dry ice, and pick-up/deliver pharmaceuticals, supplies, x-rays, lab specimens, patient records, office supplies, forms, and other items as required by the VASLCHCS. Additionally include; two (2) customer references to which you have provided similar services, and your company business size indicate which small business category for which you qualify. THIS IS NOT A REQUEST FOR PROPOSAL. NAICS Code 492110 is applicable to this acquisition, and the business size standard for a contractor is 1,500 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System Award Management System (SAM) database prior to award of a contract.
