This is a SOURCES SOUGHT notice for solicitation number 36C25822Q0007. The Contractor shall provide Adult Day Health Care (ADHC) Services to eligible beneficiaries for whom such care is specifically authorized by the Phoenix Arizona VA Health Care System (PVAHCS). The Contractor shall provide all necessary personnel, equipment, transportation, and supplies associated with the full range of ADHC services in performing the requirements. The patients to be cared for under this agreement will receive these services as a substitute for institutionalization. The Government anticipates making a multiple awards as a result of the solicitation in the following AZ Counties: Cochise, Graham, Pima, Pinal, Santa Cruz, and Yuma. The solicitation period will be open for one year from the date of release; awards will be made at the Government s sole discretion. Interested Offerors are encouraged to provide a quote as soon as possible. The solicitation will be available for download from http://www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the quote submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments.
