Page 12 of 15 Performance Work Statement Heath Aids GENERAL: Services Provided: The Contractor shall provide forty (40) Health Aids who can perform services in accordance with the specifications contained herein to beneficiaries of the Department of Veterans Affairs (VA) New Mexico VA Health Care System (NMVAHCS). The contractor shall supply all management, supervision, and labor necessary to provide non-personal staffing services. Place of Performance - Contractor shall furnish services at the New Mexico VA Health Care System (NMVAHCS) at 1501 San Pedro Dr. SE, Albuquerque, NM 87108 Period of Performance: Base period: 180-day base period with 180-day option. QUALIFICATIONS: Staff/Facility The Health Aids will need a non-PIV badge. They do not need computer access, but do need to wear the non-PIV badge, as all employees are required to wear. Technical Proficiency - Contractor s employees shall be technically proficient in the skills necessary to fulfill the government s requirements, including the ability to speak, understand, read, and write English fluently as required by 38 U.S.C. 7402(d), and 7407(d). Contractor shall provide documents upon request of the CO/COR to verify current and ongoing competency, refuse skills, certification and/or licensure related to the provision of care, treatment and/or services performed. Contractor shall provide verifiable evidence of all educational and training experiences including any gaps in educational history for all contractor s employees and contractor s employees shall be responsible for abiding by the Facility's Medical Staff By-Laws, rules, and regulations (referenced herein) that govern medical staff behavior. Uniforms: All contract personnel will provide their own uniforms and nametags and will wear them anytime the personnel are on any Department of Veterans Affairs (VA) Medical Center (VAMC) grounds. Attire is relaxed casual. No torn jeans, no sweatpants, no shorts. Physical Demands: The incumbent must be able to perform all activities of the position without restriction. Moderate physical effort (lift/carry up to 50 lbs.) Prolonged extensive, or considerable standing/walking Lifts supplies/equipment Considerable reaching, stooping, bending, kneeling, and crouching Key Personnel: The VA Full Time Equivalent Employee (FTEE) for the services required is 2,080 hours= 1.0 VA FTEE. FTEE is defined by VA as a minimum of 80 hours every two weeks and does not include holidays. The number of Health Aids is 40 FTEE located within the NMVAHCS, daily (be on site) as defined in paragraph Hours of Operation in this section. The Contractor shall be responsible for providing coverage to the VA during periods of vacancies of the Contractor s personnel due to sick leave, personal leave, vacations, and additional coverage as required. Personnel Substitutions: During the first ninety (90) calendar days of performance, the Contractor shall make NO substitutions of key personnel unless the substitution is necessitated by illness, death, or termination of employment. The Contractor shall notify the CO, in writing, within ten (10) calendar days after the occurrence of any of these events and provide the information required below. After 90 days, the Contractor shall submit the information required below to the CO at least 120 calendar days prior to making any permanent substitutions. The Contractor shall provide a detailed explanation of the circumstances necessitating the proposed substitutions, complete resumes for the proposed substitutes, and any additional information requested by the CO. Proposed substitutes shall have comparable qualifications to those of the persons being replaced. The CO will notify the Contractor within 30 calendar days after receipt of all required information of the decision on the proposed substitutes. The contract will be modified to reflect any approved changes of key personnel. For temporary substitutions where the key person shall not be reporting to work for three (3) consecutive workdays or more, the Contractor shall provide a qualified replacement for the key person. The substitute shall have comparable qualifications to the key person. Any period exceeding two weeks will require the procedure as stated above. Any replacement personnel must be credentialed and in good standing with the VA. The Government reserves the right to refuse acceptance of any Contractor personnel at any time after performance begins, if personal or professional conduct jeopardizes patient care or interferes with the regular and ordinary operation of the facility. Breaches of conduct include intoxication or debilitation resulting from drug use, theft, patient abuse, dereliction, or negligence in performing directed tasks, or other conduct resulting in formal complaints by patient or other staff members to designated Government representatives. Standards for conduct shall mirror those prescribed by current federal personnel regulations. Should the VA COS or designee show documented clinical problems or continual unprofessional behavior/actions with any Contractor s employees s/he may request, without cause, immediate replacement of said Contractor employees. The CO and COR shall deal with issues raised concerning Contractor s employees conduct. The final arbiter on questions of acceptability is the CO. Contingency Plan: Because continuity of care is an essential part of NMVAHCS s medical services, The Contractor shall have a contingency plan in place to be utilized if the Contractor s employees leave Contractor s employment or is unable to continue performance in accordance with the terms and conditions of the resulting contract. HOURS OF OPERATION: Duty Hours: The contractor will provide 40.0 FTEE Health Aids to NMVAHCS as required. Currently normal facility hours are open 24 hours per day. The contractor s employees are subject to varying work hours depending on staffing needs and may be revised as deemed appropriate by the Department Director. The Contracting Officer s Representative (COR) will provide technical direction on specific tour of duty. Typically, the tour of duty shall be: For Health Aids, Hours are 24/7, including holidays, in all weather conditions. Shifts can also be 8-hour shifts, 10-hour shifts, and 12-hour shifts. This is outdoor work, usually from 0530 1800, times subject to change, depending on current requirements. This will include Federal Holidays and rotating weekends. Federal Holidays: The following holidays are observed by the Department of Veterans Affairs: New Year s Day President s Day Martin Luther King s Birthday Memorial Day Juneteenth Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Christmas Any day specifically declared by the President of the United States to be a national holiday. CONTRACTOR RESPONSIBILITIES: Personnel Required: The Contractor shall provide contractor s employees who are competent, qualified per this performance work statement and adequately trained to perform assigned duties. Contractor s employees shall be responsible for signing in and out when in attendance. Time sheets will be used by the COR to confirm hours/day and services provided against the contractor s invoices. Qualifications: The Health Aids will need a non-PIV badge. They do not need computer access, but do need to wear the non-PIV badge, as all employees are required to wear. Attire is relaxed casual. No torn jeans, no sweatpants, no shorts. Duties: Health Aid duties include, but are not limited to: Screen/interview all employees, visitors, and veterans, entering the VA compound, consisting of one, up to 5 gate entrances. Employees must show their PIV card, Veterans have to show ID card and proof of medical appointment, and or vaccination cards, and are waved thru. All are asked if they are experiencing any symptoms. If they are, they are sent to Level 1 enhanced to evaluated further. If they are not experiencing symptoms, they can enter campus. This screening is done outdoors, sometimes under harsh weather conditions. When the facility is closed for the day, the Health Aids is to screen from the ED, which was the only point of entry to the hospital on weekdays and after 6pm. This location and hours are subject to change, depending on current requirements. They work inside the main hospital, monitoring for proper wearing of masks and monitoring children under the age of 18 were not inside the hospital. Direct traffic throughout the facility grounds. Providing directions and Stopping traffic, allowing Ambulances to come thru. Responsible for driving the electric bus to transport patients to and from San Mateo gate. Worked on the units and ED as Patient Safety Attendants, were runners in the ED, transport patients in the ED/ run labs from ED to basement and answered phones on the units. Covering front information desk to provide guidance to veterans on where to go/ calling transport companies, etc. Manning escort office to assist patients in wheelchairs or walkers/ running errands throughout the hospital campus. Adjusting signage for entry gates when hours accessible changed. I.E. from one way to 2 way at 0530 and then at 1700. They must deliver vaccination from the VA pharmacy to the specified 13 C/CBOCs located throughout the state of NM and Durango Colorado. Must support off site vaccination efforts, i.e. Border Patrol vaccinations in Artesia, American Legion in Albuquerque. They must obtain driving requirements to drive VA fleet vehicles. Contractor shall be responsible for assuring the accuracy and completeness of all reports and other documents as well as the timely submission of each. Contractor shall comply with contract requirements regarding the appropriate reporting formats, instructions, submission timetables, and technical assistance as required. The following are brief descriptions of required documents that must be submitted by Contractor: upon award; weekly; monthly; quarterly ; annually, etc. identified throughout the PWS and is provided here as a guide for Contractor convenience. If an item is within the PWS and not listed here, the Contractor remains responsible for the delivery of the item. Methods of Surveillance: In an effort to minimize the performance management burden, simplified surveillance methods shall be used by the government to evaluate contractor performance when appropriate. The primary methods of surveillance are: Direct Observation - Direct observation shall be performed periodically or through 100% surveillance. All performance standards. This method requires the Contracting Officers Representative (COR) to conduct surveillance of contractor services each time the service is performed. Results will be annotated on a surveillance checklist. Any deficiency shall be documented, and the contractor shall re-perform service as directed by COR. Any unsatisfactory surveillance results identified but re-performed acceptably shall still be counted as an unsatisfactory surveillance. Periodic Inspection - Inspections scheduled and reported quarterly per COR delegation or as needed. Five (5) randomly selected patient files will be reviewed per inspection period. All inspections and reports will be conducted in compliance with VA Privacy and Information security standards. Random Inspection Five (5) randomly selected patient files will be reviewed per quarter. All reviews and reports will be conducted in compliance with VA Privacy and Information security standards. Patient Complaints - Review of Truth Point Surveys and any patient comments. INSPECTION NOTE: The COR shall record results of inspection, noting the date & time of inspection. If inspection indicates unacceptable performance, the government evaluator shall notify the CO of the deficiencies and have contractor correct those deficiencies. Contractor shall be given notification to correct the deficiencies within a reasonable amount of time, on a case-by-case basis. The CO shall have the final authority on the amount of time the contractor has to correct the deficiency. The evaluator shall not issue a receiving report accepting the services for the month in question until all deficiencies have been corrected.
