The U.S. Department of Veterans Affairs (VA), Network Contract Office (NCO) 22-G intends to issue a solicitation for transitional housing for veteran patients of the Phoenix VA Health Care System (PVAHCS) located in Phoenix, AZ. The services shall be provided directly to the Veterans who are eligible beneficiaries specifically authorized by PVAHCS. Period of Performance: 11/01/2021 to 10/31/2022 with four (4), twelve (12) month option periods. The NAICS code for this solicitation is 623220 (Residential Mental Health and Substance Abuse Facilities) with a business size standard of $16.5M. This solicitation will be posted unrestricted. The Government anticipates awarding a firm fixed price, indefinite quantity contract. Multiple awards may result from this solicitation. The contractor shall perform in accordance with the Performance Work Statement that will be included in the solicitation.  All responsible sources may submit a quotation, which if timely received, shall be considered by this agency. Solicitation Package 36C25821Q0355 shall be posted on beta.SAM.gov on or about September 18, 2021.  No solicitation packages will be mailed.  Responses must be concise and be specifically directed to the requirement referenced above. All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. The Department of Veterans Affairs (VA), Network Contracting Office (NCO) 22 - Gilbert, is conducting market research to identify potential vendor sources which can provide Residential Housing for veteran patients with serious mental illness for Phoenix, AZ VA Healthcare System located at 650 E Indian School Road, Phoenix, AZ 85012. The Contractor shall provide housing for veteran patients on an as needed basis. Transitional housing is intended to provide short term case management services as a step up or step-down housing program. Veterans either are admitted directly off the streets or they are discharged from a pending in-patient or residential care. The contractor will be requirement to provide case management and treatment planning for veteran patients authorized in the facility. These services include therapeutic and rehabilitative services completed through referrals from the Phoenix VA facility. These services include the following: Structured group activities as appropriate – examples include group therapy, social skills training, Alcoholics Anonymous, Narcotics Anonymous, vocational counseling and physical activities as appropriate. Collaboration with the VA program staff, which will provide supportive psychosocial services. Individual professional counseling, including counseling on self-care skills, adaptive coping skills and, as appropriate, vocational rehabilitation counseling, in collaboration with VA program and community resources. Assistance to develop life skills for responsible living patterns, to maintain an acceptable level of personal hygiene and grooming, and to achieve a more adaptive level of psychosocial functioning, upgraded social skills, and improved personal relationships, maintaining personal financial responsibilities. Support for an alcohol/drug abuse-free lifestyle provided in an environment conducive to social interaction and the fullest development of the resident’s rehabilitative potential. Assistance to gain and to apply knowledge of the illness/recovery process in an environment supportive of recovery models. A program that promotes community interaction.
