The Contractor shall provide continuous delivery of primary and preventive medical care services, including tele-mental health services, for eligible Veterans enrolled with the NMVAHCS at the Contractor s clinic facilities that are to be located at the following location: Durango, CO for all assigned patients, in accordance with the terms and conditions stated in the forthcoming solicitation, on behalf of the NMVAHCS, 1501 San Pedro Drive SE, Albuquerque, New Mexico, 87108. The Contractor s clinic site shall be located within the city limits in Durango, Colorado. Contract will be for a Firm Fixed-Price Indefinite Delivery/ Indefinite Quantity. Pre-solicitation Synopsis this procurement is set-aside for Service-Disabled Veteran Owned Small Businesses. The Principle NAICS Code is 621498 Medical All Other Outpatient Care Centers. The size standard is $22 Million. The PSC is Q201 Medical - General Health Care. The RFP Number is 36C25820R0031.
