The requirement will be a Firm Fixed Price 100% Service Disabled Veteran Owned Small Business (SDVOSB) set-aside and only qualified respondents may submit information. The requirement will be ordered as needed. The maximum order quantities are listed in each year. I have also included the embroidery that we need to have on certain clothing. REQUIREMENT Uniform items will be exact match to quantities and part numbers listed in attachment Please see list ALL items must be available from the vendor. All lab coats will be in compliance with OSHA and Infection Control standards under Bloodborn Pathogen standard section (d)(3)(i), (d)(3)(ii) and (d)(3)(xi) Provide flame resistant clothing to be UL- Certified under: NFPA 2112 and Meet NFPA 70E Requirements. Services: Vendor will provide embroidery to uniforms in accordance to picture, description, measurements and location in Appendix B. The embroidery will be ordered as needed. DELIVERABLES The vendor shall provide uniforms and services listed attachments for Department of Veteran Affairs at 650 E. Indian School Road Phoenix AZ 85012 for an Indefinite delivery, indefinite quantity. Vendors are required to be VIP verified SDVOSB s only. Only vendors from firms that are certified to meet Small business size standard will be considered.
