The VA Medical Center located in Dallas, TX is needing an Alcon Clinical and Technical Service contract for Alcon Constellation LXT, LenSX, and Centurion Ophthalmology equipment. The contractor shall provide all services as listed in the equipment description below, meeting all technical specifications as seen, for the purpose of maintaining and supporting the Alcon Constellation LXT, LenSX, and Centurion Ophthalmology equipment. The contractor shall provide all required services, labor, travel, and parts for VA software and equipment listed below in accordance with the original equipment manufacturer specification. All parts used must be approved by the FDA where applicable and be sourced directly from the OEM. All services provided must be performed by OEM-certified personnel. Contractor shall provide a complete service report upon delivery of any service calls to Biomedical Staff in the Biomed Department. The full-service contract will cover one (1) Base year and 3 option years with a start date of February 2, 2025 through January 31, 2026. Responses to this notice shall include company/individual name, size status for the above reference NAICS code, a service capability statement, SAM Entity Unique Identifier (EUI), DUNS Number, address, point of contact and examples of similar facilities to which similar services have been provided to. Contractors should provide information on whether they are certified SDVOSB, VOSB, Hub zone, 8(a), women-owned concern.
