DESCRIPTION The Department of Veterans Affairs (VA), Veterans Health Administration, Veterans Integrated Service Network 17, Network Contracting Office (NCO) 17 is seeking sources for the following procurement: Ultrasound and Laser Service. Description of Services: Vendor Responsibilities for Urology and ENT Cases The vendor agrees to transport and provide all essential equipment, products, and services required for Urology and ENT. The products and equipment are as follows; Holmium High Watt (greater than 30 watts), Laser Enucleation System (Piranha), Ultrasound-including probes, Ultrasound Including probes, Holmium Master Pulse, Flexible CO2 Laser, OmniGuide Handpiece Set OTO, Imperium 300 OTO Fiber. The Vendor shall be notified forty-eight hours prior to each case to ensure adequate coverage is maintained. Urology and Ent procedures shall be conducted at the following locations: 1. Audie L Murphy VA Medical Center Address: 7400 Merton Minter Blvd, San Antonio, TX 78229 2. Christus Santa Rosa Hospital Address: 403 Treeline Park, San Antonio, TX 78209 The vendor is accountable for strict compliance with the notification timeline and ensuring the smooth delivery of equipment, products, and services for urology and ENT cases, including but not limited to Holmium Laser Enucleations of the Prostate, Ureter procedures, Robotic partial Nephrectomies, and Stapes procedures at the specified locations. a. During the term of agreement, the equipment will be utilized only for medically necessary surgical procedures ordered by a licensed physician. The equipment will be used solely for its intended use, consistent with the Equipment s labeling and in compliance with applicable standards of care, regulations, and laws. Each Surgeon designated to operate the equipment is trained and competent in the use of the equipment in his or her surgical specialty. The surgeon staffing the case will be VA Personnel. The vendor will provide a trained and competent technician to assist will all cases being performed. Vendor is responsible for any troubleshooting or maintenance of all equipment provided. b. Vendor will provide on-site setup at the service location indicated by the assigned COR Lancelot Ellis. Delivery will occur during normal business hours M-F from 0630 to 1500. Deliver/installation all communication will be coordinated with the assigned COR.See referenced attachments for description of work and equipment list: SOW Ultrasound and Laser Service Period of Performance: 07/27/2024-07/26/2029 NCO 17 is seeking sources that have the capability of providing these services. Firms that can provide these services are encouraged to respond to the sources sought notice by contacting the point of contact. Capable firms must respond to the POC no later than 12PM CST on June 3, 2024, to be considered. Interested firms must identify their company name, SAM UEI number, socioeconomic category, and are asked to provide a capabilities statement with their response. This notice is NOT a formal request for quotes or proposals. No formal solicitation document exists currently. The information in this notice is for information and planning purposes only and is not to be construed as a commitment by the government. No contract will be awarded from this notice. The acquisition strategy will be decided based on responses and other market research efforts. If parties choose to respond, any cost associated with the preparation and submission of data or cost incurred in response to this announcement shall be the sole responsibility of the contractor and will not be reimbursed by the Government.
