The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources, small and other than small businesses, to perform a potential requirement. Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 532490 and the size standard is $35.0M. Project Requirements: The Contractor shall provide rental of Urology Equipment and Technician Services on an as needed basis for the Department of Veterans Affairs, Central Texas Veterans Health Care System (CTVHCS), 1901 Veterans Memorial Drive, Temple, TX 76504 in accordance with terms and conditions specified herein. Place of Performance: Services shall be performed at the Department of Veterans Affairs, Central Texas Veterans Health Care System (CTVHCS), 1901 Veterans Memorial Drive, Temple, TX 76504 in accordance with terms and conditions specified herein. Anticipated period of performance: Base plus four (4) one-year options. Synopsis General: This is a non-personnel services contract to provide Autopsy Services. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. Services Provided: The Contractor shall provide rental of Urology Equipment and Technician Services as specified in the statement of work contained herein on an as needed basis for the Department of Veterans Affairs, Central Texas Veterans Health Care System (CTVHCS), 1901 Veterans Memorial Drive, Temple, TX 76504 in accordance with terms and conditions specified herein. The Contractor’s Technician shall be certified to operate the required equipment by the equipment manufacturer. The Contractor shall ensure that all qualifying criteria outlined in this section have been met, to include current and valid licenses and certifications. The Contractor shall be required to maintain documentation and provide copies, with their proposal, of the following for each Technician working under the contract: Qualifications, Certifications, record of credentials and competencies Applicable License(s), including number and expiration date Documentation of education and training Infection Control testing results, to include.
