The purpose of this amendment is to provide a second date for completion of a site visit and to extend the solicitation close date as follows: 1. Additional site visit date will be 9/7/21 at 9:00 AM CST, see Section A.1 below for details. 2. Questions due date has been extended from 8/30 to 9/9 @ 4:00 PM CST. 3. Solicitation close date has been extended from 9/3 to 9/13 @ 3:00PM CST. All other terms and conditions remain the same. A.1 SITE VISIT INFORMATION Site visit will be held on Tuesday, September 7th 2021 at 9:00 AM CST at the location specified in the Delivery Schedule. Vendors are to meet at the front desk in the lobby. Upon building entry there will be COVID screening and then each vendor will receive a badge. Please see attachment 3 of original solicitation, AUSH Visitation Memorandum for more COVID details and be sure to contact the site prior to visit as COVID requirements may change.
