THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. VHA RPO West NCO 17 Health Care Resources Branch I is planning to solicit for a vendor who can provide Ophthalmology Retina Specialty Medical Services for Veterans within the area of the El Paso VA Health Care System. The Eye Clinic at the El Paso VA needs a Contractor that can provide on-site Retina Specialty services on Tuesdays and Wednesday from 8:00 AM to 4:45 PM for a total of 16 hours a week. Contractor shall have an off-site location to provide emergency and surgery care. VHA RPO West NCO 17 Health Care Resources Branch I is conducting a market survey to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to fulfill this acquisition. Place of Performance: Department of Veterans Affairs El Paso VA Health Care System 5001 N Piedras Street El Paso, TX 79930 Type of Contract: If this request for information (RFI) leads to a future solicitation, the Government intends to award a firm-fixed price contract. The performance period shall be one (1) base year with four (4); one (1) option years. Interested parties shall provide a brief overview of their firm's capabilities and past experience with projects similar in scope to this project. All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each capability statement submittal shall also include the following business information: a. Organization Name b. Organization Address c. Organization DUNS Number d. Point of Contact (including name, title, addresses, telephone number, fax number and Email address) e. Business size under NAICS 621111 f. Socio-economic status (IF SDVOSB or VOSB - provide SDVOSB or VOSB CVE verification) g. Location of off-site location(s) and how far they are from the El Paso VA. Firms shall identify whether or not they are a small business or other than small business in accordance with the size standard for NAICS code 621111. If another NAICS code may be applicable, please identify the NAICS in your capability statement. All information must be received no later than January 8, 2021 by 2:00 PM ET.
