The purpose of this Request for Information is to determine interest and capability of businesses including Service-Disabled Veteran-Owned Small Business, and Veteran-Owned Small Business firms relative to the North American Industry Classification Code (NAICS) 621991. The Department of Veterans Affairs is seeking both manufacturers and/or distributors of the exact match or functionally equivalent equipment/services to the following listed above. Teague VA Hospital is seeking to purchase CORNEAL BIOLOGICAL GRAFTS. The contactor shall furnish corneal biological grafts to corneal surgery cases at Olin E. Teague VA Hospital, Eye Clinic/Ophthalmology referred to as the VA. Contractor/vendor is to supply corneal biological grafts in the quantity stated within this SOW for eye surgery cases. No technician services or additional equipment or supplies is required from the contractor. Estimated eye transplants performed may vary. VA is not committed to a set number of grafts and is invoiced by the contractor/vendor per requested graft. 2.CONTRACTOR REQUIREMENTS The contractor shall provide the biologicals specified in this contract, which may be changed only by written modification to this contract. This contract is for the purchase of a commodity only, which includes shipment of the corneal grafts. One-year contract 2020 2021. No other services are required from the contractor. Any subcontractor used by the Contractor for the provision of products required under this contract shall meet the same qualifications specified herein for the Contractor. Contractor shall provide the grafts during normal operating hours, which should include hours of operation 8:00 am 4:30 pm on Monday through Friday, while observing federal holidays. Any additional holidays authorized by Presidential Executive Order will be announced. Current observed federal holidays are: New Year s Day Martin Luther King s Birthday Presidents Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day Any other day specifically declared by the President of the United States
