All information received in response to this RFI that is marked as proprietary will be handled accordingly. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. Your response to this SSN will be treated only as information for the Government to consider. You will not be entitled to payment for direct or indirect costs that you incur in responding to this SSN. This request does not constitute a solicitation for proposals or the authority to enter into negotiations to award a contract. No funds have been authorized, appropriated or received for this effort. Interested parties are responsible for adequately marking proprietary, restricted or competition sensitive information contained in their response. The Government does not intend to pay for the information submitted in response to this SSN SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is an RFI/sources sought only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to North American Industry Classification System (NAICS) 811210 (size standard of $34.0M). Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 16 Network Contracting Office, is seeking sources that can provide the following for the Michael E. DeBakey VA Medical Center: Requesting for a contractor to provide preventative maintenance and service on the Histology lab equipment at the Michael E. DeBakey Medical Center located at 2002 Holcombe Blvd, Houston, TX 77030. The Contractor shall furnish all labor, travel, materials, parts, equipment, tools to service and perform yearly preventative maintenance on the equipment. The Contractor must provide 24/7 technical support via phone, email and web. In the event the Contractor must replace any part, the part must be replaced with certified OEM replacement parts. See attached DRAFT Statement of Work. The information identified above are intended to be descriptive, not restrictive, and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications. If you are interested and are capable of providing the sought-out services, please provide the requested information as well as the information indicated below. Socioeconomic status (whether Service Disable Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), HUBZone, 8(a), Woman Owned, Small Disadvantaged, etc.). If stating SDVOSB or VOSB status, your company must be VetCert certified by SBA.*** Is your company considered small under the NAICS code identified under this RFI? Are you the manufacturer, an authorized distributor, or an equivalent solution to the services/parts being referenced above? If an authorized distributor/service provider, provide proof of authorization. How long has your company provided these products and/or services? Provide information for any current or past VA contracts for these products and/or services. Does your company have an FSS contract with GSA, VA NAC, or any other federal contract, that can be utilized to procure the requirement listed above? If so, please provide the contract number. Submit your capabilities that show clear, compelling, and convincing evidence that you can meet the requirement described in the attached SOW. Provide your UEI number.
