The agency is seeking any and all interested, eligible and qualified sources (incl. small business concerns and other than small business). This announcement seeks information from the industry, which will be used for preliminary planning purposes. No proposals are being requested or accepted with his synopsis. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS SYNOPSIS. Respondents will not be notified of the results of this synopsis. THE PROJECT REQUIREMENTS: LONG TERM CARE MOCK SURVEY for the Southeast Louisiana Veterans Health Care System (SLVHCS), 2400 Canal Street, New Orleans, LA 70119. Therefore, it is required that this Services be performed by trained and certified personnel. As the current contract is expiring, a new service agreement is needed to continue these services. Contractor shall furnish all Services to support and provide the following: LONG TERM CARE Mock Survey Scope of Work The assessments should cover all aspects of inspection preparedness within the scope of the services provided in the Community Living Center of Southeast Louisiana Veterans Health Care System. 1. The contractor shall utilize individual and system tracers for the evaluation of determined areas of focus which include but are not limited to: privacy and dignity pain management use of antipsychotic medications prevention of pressure ulcers reduction of fall risk reduction of indwelling urinary catheter uses and reduction of catheter associated urinary tract infections medication administration 2. The contractor shall provide feedback during the assessments and a summary at the end of the survey to include findings scored on a scoring system similar to that of CMS Scope and Severity Index. 3. Additional information to assess the status of the organization s inspection readiness may be collected during the mock survey via: Staff and Veteran interviews Review of: policies and standard operating procedures committee meeting minutes risk assessments and plans 4.
