Page 1 of 3 STATEMENT OF WORK (SOW) Contract Title: Ophthalmology Eye Lane, Chair and Slit Lamp Background: Exam Lane Combo (Exam chair, instrument stand, slit lamp) to support new Ophthalmologist at Communicty Based Outpatient Clinic (CBOC), Joint Ambulatory Care Center (JACC). Requirement for equipment delivered to Biloxi and then installation at the JACC in Pensacola, FL. Scope: Surgery Service Eye section is requesting one (1) exam lane combo in accordance with specifications and characteristics. Specifications/Characteristics: See CLINS for equipment requirements. Delivery. Delivery address: Gulf Coast Veterans Healthcare System 400 Veterans Avenue Bldg. 26 Warehouse Biloxi, MS 39531 Mon-Fri 7:00am- 2:30pm Installation: Vendor shall coordinate installation date and time with the Program Manager, Tiffanie Williams. Equipment is to be shipped and delivered to the Gulf Coast Veterans Healthcare System in Biloxi, MS for the Biomed to tag and inventory the medical equipment. Vendor shall coordinate w/ Ms. Williams the date and times for equipment to be transported to the CBOC, JACC to intall and establish the Eye Lane to be fully functional/operational for service. Installation shall be performed between Monday Friday, excluding Federal Holidays.
