Statement of Work Transport and Disposal of Hazardous Waste and Non-Hazardous Pollutants BACKGROUND: Department of Veterans Affairs, Gulf Coast Veteran s Health Care System (GCVHCS) is seeking removal services of waste products (hazardous and non-hazardous, excluding waste pharmaceuticals) and associated contaminated materials for 12 months plus 4 option years for the main campuses, Joint Ambulatory Care Centers, associated Outpatient Clinics, and Community Based Outpatient Clinics (CBOC). SCOPE: The contractor shall provide all labor and materials necessary for the collection and removal of waste products (both hazardous and non-hazardous, excluding waste pharmaceuticals) and associated contaminated materials for the Department of Veterans Affairs; Gulf Coast Veteran s Health Care System (GCVHCS) . waste products (both hazardous and non-hazardous, excluding waste pharmaceuticals) and associated contaminated materials shall be processed and disposed of off-site by contractor at a facility in compliance with all Federal, State and Local requirements. Management and processing of wastes and completion of all services shall be in accordance with all Federal, State, and Local regulations. WASTE REMOVAL SERVICES: waste includes waste products (both hazardous and non-hazardous, excluding waste pharmaceuticals) and associated contaminated materials. Contractor shall: Provide the appropriate containers, liners, shipping papers, pick-up, transportation, processing and tracking of waste products (both hazardous and non-hazardous, excluding waste pharmaceuticals) and associated contaminated materials. Waste(s) will typically include: spent solvents from laboratory (ethanol/xylene mixture) reagent cassettes from automatic analyzers containing corrosive liquid. Expired (unused) formalin and methanol in specimen containers Used and expired Phenol swabs. Silver nitrate used and expired applicators Expired laboratory reagents (corrosives, TCLP toxics, solvents, ignitable liquids, limited reactives (typically cyanides, sulfide compounds, zinc metal). Pads contaminated with oil/diesel/petroleum. Barium Sulfate Contrast Medium for X-Rays Universal Waste(s) typically include: Intact Fluorescent light tubes. Broken Fluorescent light tubes. Batteries for recycle [Lead Acid Batteries (D Cell or smaller), Nickle Cadmium, Nickle Metal Hydride, Silver, Mercury, Alkaline, Zinc Air]. Note: Large Lead Acid Batteries are screened out and recycled locally by the facility and are excluded from this contract. Mercury containing equipment Pesticides Ensure provided containers are leak-proof, stackable, lidded, shall display type of waste and all other US DOT and state required marking to ensure compliance with all transportation requirements. All containers shall always be damage free and in good working order. Ensure provided containers are clearly identifiable for the regulated waste streams. Remove full containers and replace with an equal or greater supply of disposable containers and liners for collection. . Ensure prior to departing any facility, contractor has obtained the COR or designee s signature for confirmation of manifest including but not limited to: Pickup/Delivery Location, Pickup/Delivery Date, Proper Description of the Waste, Volume of Waste (number of containers picked up and/or pounds), US DOT Compliant Manifest with Unique Number (Shipping Papers). Transport the waste to the treatment facility. Treatment shall be by, recycling, reclamation, incineration, or other means in accordance with all applicable Federal, State and Local requirements. Transport and Disposal of waste The Contractor shall transport and dispose of the waste in accordance with all applicable Environmental Protection Agency (EPA), U.S. Department of Transportation (DOT), Mississippi Department of Environmental Quality (MDEQ), Occupational Safety and Health Administration (OSHA), and any other state and federal guidelines. NOTES: DOCUMENTATION FROM THE CONTRACTOR: The contractor shall prepare and submit to the COR an original signed Certificate of Destruction (manifest) for each pick up for each site. The contractor shall maintain proper records concerning each Certificate of Destruction issued. The manifest shall at a minimum contain the following: Pickup/Delivery Location, Pickup/Delivery Date, Amount of waste (number of containers picked up and/or pounds), Destruction Certification Document/Manifest Number, Destruction Certification Date, any other information as required by the COR. Completed manifests confirming delivery and processing of the regulated wastes shall be submitted within 35 days of initial pickup date. Contractor shall provide monthly reporting that shows the pounds by waste stream shipped and processed to the COR. This shall be tracked each month and provided in excel spreadsheet. Waste Classification and determination: The contractor will provide designated contact information for submission of documentation demonstrating generator s knowledge and/or analytical results as appropriate for wastes offered for removal/disposal under the contract statement of work. Wastes that are generated on a recurring basis will need to be profiled annually. Working with the COR the contractor will provide training per the SOW based on the classification of the wastes identified during review of profiled wastes and interview of GEMS manager. TRAINING FOR CUSTOMER: Contractor shall provide annual training to government staff as required by the COR. Training shall be for: US DOT HAZMAT according to 49 CFR 17d Sub Part H employees. Training shall cover General Awareness/Familiarization, Function Specific (including bulk outer packaging), Safety, Security Awareness. Training shall be ON SITE at the VA facilities and cover all shifts. Training shall occur over three days annually and will be Coordinated and scheduled by the COR and Vendor. Resource Conservation and Recovery Act (RCRA) according to 40 CFR 262.16(b)(9)(iii). Training shall cover introduction, identification, accumulation, pre-shipment requirements, manifesting, disposal, recordkeeping, and emergency procedures as pertains to hazardous wastes. TRAINING FOR CONTRACTOR: All contractor on-site employees and on-site subcontractors under this contract or order are required to complete the VA's on-line Combined Privacy and Information Security Awareness Training Course and the additional Privacy Awareness Training Course annually. Contractors must provide signed certifications of completion to the COR every 12 months during each year of the contract. This requirement is in addition to any other training that may be required of the contractor and subcontractor(s). Containers: Department of Veterans Affairs requires all waste containers > 5 Gal., to be on either dollies or carts and not sitting directly on the floor. Contractor shall provide clean replacement containers as needed on or before scheduled service days Pick Up Location: The COR is responsible to provide an initial drawing for GCVHCS and each CBOC and Outpatient Clinic identifying the correct location of pick-up locations. This shall be updated quarterly to ensure it is always as accurate as possible. QUALITY CONTROL: Contractor must replace all containers that are more than approximately three-quarters (3/4) full, to ensure no overfills between service days. Contractor should have service infrastructure to handle all applicable issues related to waste disposal and service. Contractor should be able to complete all terms and conditions of the contract immediately following the award of the contract. There are to be no delays in delivering the service and/or products. CONTRACTOR S RESPONSIBILITY: Contractor will only transport the waste to facilities that are permitted for the processing of each waste. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to their employees, or others, as well as for any damages to personal or public property that occurs during performance of this contract that is caused by their employees' fault or negligence. 13. QUALITY ASSURANCE: The Government will investigate complaints from any facility. All information received will be provided either verbally and/or in writing to the contractor for appropriate action. 14. WORK HOURS: All pickups shall be performed between 8:30am and 3:30pm, Monday Friday or as established by the Facility COR/Point of Contact 15. NATIONAL HOLIDAYS: Holidays observed by the Federal Government are: - New Year s Day - Martin Luther King s Birthday - President s Day - Memorial Day - Juneteenth Day - Independence Day - Labor Day - Columbus Day - Veteran s Day - Thanksgiving Day - Christmas Day 25. HOLIDAY RESCHEDULING: Generally, when it is necessary to reschedule a service and/or pickup because of a holiday; service and/or pickup must occur on the next business day following the holiday (alternate arrangements can be made with individual sites at their discretion). 26. PLACE OF PERFORMANCE: Gul Coast Veteran s Health Care System and its associated Outpatient/Outreach Clinics.
