Consistent with VISN policy, the existing audiology clinic has been providing comprehensive audiologic services within the practice scope of an audiologist for veterans. This included comprehensive audiologic (hearing) evaluation, tinnitus, dizziness and balance evaluation, cerumen removal, hearing aid and other prostheses consultation and dispensing program and a hearing aid repair lab to support walk-in hearing aid services. Purpose: The purpose of this request is to acquire both equipment, components, software, supplies, and vendor services to ensure a functioning audiology clinic will be fully operational to serve veterans at 180 East Sunbridge Drive. Acquisition for equipment, supplies, and vendor services will be based on the above scope of services planned. Performance Scope: Performance criteria 1: Diagnostic hearing assessment equipment shall be fully assembled and set up at the stated location. The contractor shall furnish, ship, and transport all supplies, equipment, components, ancillary parts, and software. The contractor is responsible for any missing parts and components not included in order to carry out the installation and assembly to its full extent (i.e. equipment shall function upon turning the power on and the landing page displayed.) All equipment shall be new and not refurbished. All equipment and installation must meet manufacturer and VA specifications. All products must meet the salient characteristics inherent to the equipment. All equipment must also function according to manufacturers stated specification and functional capacity/capability. All equipment functional standards must conform to ANSI standards (when appropriate) and manufacturers specification.
