SOURCES SOUGHT NOTICE Community Nursing Home Services Veteran s Healthcare System of the Ozarks Fayetteville, AR DESCRIPTION: This is a Sources Sought/Request for Information to determine the availability of potential sources having the skills and capabilities necessary to provide the appropriate level of Community Nursing Home (CNH) care to veterans and eligible beneficiaries of the Department of Veterans Affairs, Veterans Healthcare System of the Ozarks (VHSO) located at 1100 N. College Avenue, Fayetteville, AR 72703. Requirement is for Community Nursing Home Services in Newton County, AR. Five Year Ordering Period with an estimated period of performance will be from April 1, 2022 through March 31, 2027. The number of veterans requiring Community Nursing Home services (bed days) over the next five years will fluctuate and cannot be adequately determined at this time. Award of a Fixed Unit Price Indefinite Delivery Contract (IDC) with Economic Price Adjustment is anticipated. REQUIRED: Interested agencies will be Medicaid approved as applicable to the location as a Certified Nursing Home Facility and licensed as a Nursing Home by the State of Arkansas in which its provider number is based. All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a source sought notice. The purpose of this notice is to gain knowledge, in this order, of potential qualified sources and their size classifications relative to NAICS 623110 Nursing Care Facilities (size standard $30.0 Mil): (Service Disabled Veteran Owned/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this source s sought synopsis are not considered adequate responses to the potential solicitation announcement. All interested offerors will be required to respond to the solicitation announcement in addition to responding to this source sought announcement. REQUESTED INFORMATION: STATEMENT OF CABABILITY: Submit a brief description, five (5) pages or less, that demonstrates how your company can provide the requested services at time of award. Include your company s experience in performing these services to the VA, other Government (Federal or State) agencies, or for a private facility.
