This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Items,” and FAR 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. Contractor shall furnish including but not limited to all labor, materials, parts, equipment, tools, transportation, and supervision to provide the Alexandria VA Medical Center, 2495 Shreveport Hwy, Pineville, Louisiana 71360 for LuxOR Ophthalmic Microscope equipment (Manufacturer: ALCON Laboratories, Inc) for expansion of the eye clinic, brand name or equal basis. The equipment, all equipment accessories, and supplies shall be brand new. The equipment and accessories are hereinafter collectively referred as “Deliverables”. All Deliverables shall be delivered 45 days from date of award and as free on board (FOB) destination. Reference the attach document for full RFQ. RFQ extended through 9.21.2021.
