Page 3 of 3 Sources Sought Notice 1.Title: Medical Beds 2.Purpose: This Sources Sought/Market Survey Notice is being conducted by the Department of Veterans Affairs, Network Contracting Office (NCO) 16 in support of the Central Arkansas Veterans Health Care System (CAVHS) located in North Little Rock, Arkansas to identify other sources that are capable of providing NEW Bariatric Beds. 3.Objective: To find interested vendors with the capability to provide Medical Beds as listed in the equipment schedule below: Equipment Schedule: Manufacturer Part No. Description: Sizewise 45060150 Sizewise Bari-Rehab Platform3 4. The North American Industry Classification (NAICS) code for this acquisition is 339113- Surgical Appliance and Supplies Manufacturing. Standard in number of employees is 750. 5. All questions shall be answered in Section 7 for the government considered contractor with the capability to meet the government requirement. 6. Responses Required: The CAVHS requires all the following questions answered in this sources sought notice. Answers that are not provided shall be considered non-responsive to the Sources Sought/Market Survey Notice and Contractor shall not be considered as part of the market research. Telephone requests will not be honored. No award will be made based on quotes or offers received in response to this notice. All responsible sources may identify their interest to meet this requirement to Jeneice McClinton, jeneice.mcclinton@va.gov by 12:00 pm (Central Time) on April 23, 2021. Additionally, any response to this notice must show clear, compelling and convincing evidence that competition will be advantageous to the government by providing the following: 7.1. Business Size and Socio-Economic Status: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a Vendor Information Pages (VIP) verified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm.
