*****SAM-DESCRIPTION: RFI-SOURCES SOUGHT' ***** Statement of Work Date 2/22/2021 Job Name Small Fragment Set Requested by Alexandria VA HCS SPS From I. Summary The VA Health Care System (VAHCS), Alexandria, LA SPS requires the Small Fragment surgical instruments for the operating room due no second set of instruments on hand. This is a safety hazard for the patient as not having equipment sterile and ready could be detrimental. The Small Fragment instruments will be housed in the Alexandria VAMC Sterile Storage. All listed items have been purchased before by other VA Sterile Processing Services throughout other VA locations. II. Objective The purpose is to procure Small Fragment instruments to allow for improved surgical procedures for veterans in the OR Areas. III. Project Scope Due to surgical instruments, installation not required. All manufacturer parts for connection to the facility s services will be included in the purchase. Customer is responsible for all costs of any additional parts, adapters, and/or material, if any, that are not Equipment and required to install the Equipment. III. Schedule Items need to be delivered ASAP. Contractor shall provide a projected schedule, in terms of days from the time of award addressing (at a minimum) the following milestones: verification that all quantities and configurations are accurate; delivery to location. IV. Salient Characteristics-Brand Name 1 SET- SMALL FRAGMENT LCP® INSTR/IMPL SET WITH SLF-TPNG SCREWS V. ISO Requirements The C&A requirements do not apply; therefore, a Security Accreditation Package is not required.
