Page 3 of 3 Sources Sought Notice 1.Title: VISTA Pocket Scanners 2.Purpose: This Sources Sought/Market Survey Notice is being conducted by the Department of Veterans Affairs, Network Contracting Office (NCO) 16 in support of the Central Arkansas Veterans Health Care System (CAVHS) located in North Little Rock, Arkansas to identify other sources that are capable of providing and delivering VISTA Pocket Scanners. 3.Objective: To find interested vendors with the capability to provide brand name or equal equipment listed below: MANUFACTURER: Manufacturing System Services, Inc. MODELS: Product # M040AC VistA Pocket Scanner 4. The North American Industry Classification (NAICS) code for this acquisition is 334290- Other Communications Equipment Manufacturing. Standard in number of employees is 750. Contractor shall be an authorized vendor, authorized by Manufacturing System Services, Inc., for proprietary equipment and services to meet all government requirements and objectives. All questions shall be answered in Section 5 for the government considered contractor with the capability to meet the government requirement. 4. Responses Required: The CAVHS requires all the following questions answered in this sources sought notice. Telephone requests will not be honored. No award will be made based on quotes or offers received in response to this notice. All responsible sources may identify their interest to meet this requirement to Jeneice McClinton, jeneice.mcclinton@va.gov by 12:00 pm (Central Time), March 19, 2021. Additionally, any response to this notice must show clear, compelling and convincing evidence that competition will be advantageous to the government by providing the following: 5.1. Business Size and Socio-Economic Status: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a Vendor Information Pages (VIP) verified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the System for Award Management (SAM) at www.sam.gov and/or the VetBiz Registry at www.vetbiz.gov . Answers that are not provided shall be considered non-responsive to the Sources Sought/Market Survey Notice and Contractor shall not be considered as part of the market research. 5.2 Are you a Manufacturing System Services, Inc., authorized vendor or authorized distributor for providing equipment and delivery of medical equipment of VISTA pocket scanners and capable to meet all government requirements and objectives? (Contractor shall attach an authorization letter or other documents from the Original Equipment Manufacturer (OEM), such that the OEM s software licensing, warranty and service provided and maintained by the OEM to be reviewed by the government to be considered as part of the Market Research and responsive to this Sources Sought notice). 5.3. Do you have an existing Blanket Purchase Agreement (BPA), General Services Administration Schedules (GSA) or Veteran Affairs (VA) Federal Supply Schedule (FSS) contract? If yes, please include the contract details and a brief summary of the products and services provided in your response. 5.4. Is your company capable of meeting all software licensing, warranty and service associated with the equipment and/or services such that the system shall be in accordance with the OEM terms and conditions? Statement of Capability: Provide a short summary of your potential approach to this type of contract and meeting the specific requirements, delivery and your experience managing similar contracts with similar requirements for the CAVHS. 6.
