Sources Sought Notice 1.Title: MASS Channel Air Endoscope Cabinet/ Scope Storage 2.Purpose: This Sources Sought/Market Survey Notice is being conducted by the Department of Veterans Affairs, Network Contracting Office (NCO) 16 in support of the Central Arkansas Veterans Health Care System (CAVHS) located in North Little Rock, Arkansas to identify other sources that are capable of providing and delivering MASS Channel Air Endoscope Cabinets. 3.Objective: To find interested vendors with the capability to provide and deliver the equipment listed below: MANUFACTURER: MASS Medical Storage MODELS: DAS-9S-LHG-HA, DAS-truConnect-Duo1 (see equipment list in draft statement of work) 4. The North American Industry Classification (NAICS) code for this acquisition is 334519-Oter Measuring and Controlling Device Manufacturing. Standard in number of employees is 500. Contractor shall be an authorized vendor, authorized by MASS MEDICAL STORAGE, LLC., for proprietary equipment and services to meet all government requirements and objectives. All questions shall be answered in Section 5 for the government considered contractor with the capability to meet the government requirement. 4. Responses Required: The CAVHS requires all the following questions answered in this sources sought notice. Telephone requests will not be honored. No award will be made based on quotes or offers received in response to this notice. Additionally, any response to this notice must show clear, compelling and convincing evidence that competition will be advantageous to the government by providing the following: 5.1. Business Size and Socio-Economic Status: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a Vendor Information Pages (VIP) verified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. Answers that are not provided shall be considered non-responsive to the Sources Sought/Market Survey Notice and Contractor shall not be considered as part of the market research. 5.2 Are you a MASS MEDICAL STORAGE, LLC., authorized vendor or authorized distributor for providing equipment and delivery of medical equipment of scope storage cabinets and capable to meet all government requirements and objectives? (Contractor shall attach an authorization letter or other documents from the Original Equipment Manufacturer (OEM), such that the OEM s warranty and service provided and maintained by the OEM to be reviewed by the government to be considered as part of the Market Research and responsive to this Sources Sought notice). 5.3. Do you have an existing Blanket Purchase Agreement BPA, General Services Administration Schedules GSA or Veteran Affairs (VA) Federal Supply Schedule contract? If yes, please include the contract details and a brief summary of the products and services provided in your response. 5.4. Is your company capable of meeting the shipping/delivery request per the Draft SOW? Statement of Capability: Provide a short summary of your potential approach to this type of contract and meeting the specific requirements per the draft Statement of Work and your experience managing similar contracts with similar requirements for the CAVHS. 6.
