The purpose of this sources sought announcement is for market research to make the appropriate acquisition decisions and to gain knowledge of potential qualified sources and their Small Business Administration (SBA) size classifications in relation to North American Industry Classification System (NAICS) 621511. The Department of Veterans Affairs, NCO 16 is seeking a source to perform Afirma Thyroid GEC testing on collected samples. Contractor should submit results to P7LMS, CAVHS by fax and all lab reports shall contain the following: Patients Full name Date of Birth Type of test ordered Type of Specimen Physician Name Date Test Ordered Date and time test reported Test Results Reference Range CAVHS Accession Number Date and time specimen collected Requesting facility name Testing Facility name and Address Contractor shall provide the COR the methodology and ranges for results on Afirma testing. Contractor shall provide College of American Pathologist (CAP) and Clinical Laboratory Improvement Amendments (CLA) (Public LAW 10-578) certifications. All interested parties who can to perform Afirma Thyroid GEC testing on collected samples shall provide a written response to this notice to include the following: Company name Address Point of Contact Phone Number DUNS Number Cage Code Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued.
