Special Notice Special Notice Page 2 of 2 Special Notice *= Required Field Special Notice Page 1 of 2 PLEASE READ THIS NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE PUBLISHED The Department of Veterans Affairs (VA) Veterans Health Administration (VHA) intends to award a sole source contract to Karl Storz Endoscopy-America, Inc., 2151 Grand Avenue, El Segundo, CA 90245 located at the Central Arkansas VA Healthcare System 4300 W. 7th St, Little Rock, AR 72205. VA facilities have a requirement for a contractor to provide preventative maintenance services for one year on the Karl Storz Modulith SLX-F2 with serial number OR831. The preventative maintenance shall include two preventative maintenance visits per year, technical phone support, and 5% off Parts and Labor excluded under preventative maintenance services. Based on market research and previous acquisition history, it has been determined that Karl Storz is the only vendor capable of providing these services and has provided the VA with a Sole Source letter stating Karl Storz Lithotripsy-America (KSLA) is the sole authorized distributor and service entity in North America for all MODULITH Lithotripters models SLX-T, SLX-F2 and SLK manufactured by STORZ Medical AG. KSLA does not authorize third party service groups to service or maintain STORZ brand equipment. This notice will be distributed solely through this website. Interested parties are responsible for monitoring this site to ensure they have the most up-to-date information about this acquisition. The North American Classification System Code is 811219, Other Electronic and Precision Equipment Repair and Maintenance. The Government intends to negotiate with only one source under the authority of VHAPM Part 813.106 Simplified Acquisition Procedures.
