The Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office (NCO) 16, in Fayetteville, AR is seeking potential sources capable of Removal & Disposal of Old IV Hoods and Installation & Certification of New Germfree Hoods for the G.V. (Sonny) Montgomery VAMC located in Jackson, Mississippi. The proposed NAICS code for this requirement is 811219, with a small business size standard of $22.0 Million. The attached Statement of Work (DRAFT) outlines the work to be performed. In accordance with the Veterans Health Administration Procurement Manual, any solicitation for these services will include the following language from VA Acquisition Regulation clause 852.212-72 Gray Market and Counterfeit Items: (c) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM or by an intermediary distributor authorized by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. If your organization is capable of meeting the requirements of the Statement of Work, and is an authorized distributor, dealer, or retailer of this specific brand of equipment (GermFree Laboratories, Inc.) you may indicate your interest in providing these services by providing the following information: Business Name Point of Contact DUNS Number Capability Statement (no more than 2 pages) Documentation FROM THE MANUFACTURER confirming you are an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts).
