Description of Requirements: The Veterans Health Administration Network Contracting Office 16, Houston, Texas, is seeking potential sources to provide Optician services in support of The Michael E. DeBakey VA Medical Center (MEDVAMC) and affiliated Community Based Outpatient Clinics (CBOCs) within the service area of Houston, Texas. The potential source will provide certified and/or registered Opticians to the MEDVAMC and CBOCs for the purpose of measuring, fitting and submitting consults of all Veteran patient prescriptions provided by the doctors in the respective Optometry and Ophthalmology clinics. Responsible sources are encouraged to submit a response to this notice with a statement of interest on a company letterhead. At a minimum, the following information shall be provided: 1. Socio-Economic (i.e. Small/Large Business, HUBZone, Service-Disabled Veteran Owned, 8(a), etc.) as it relates to the applicable NAICS code is 561320 entitled Temproary Help Services, with a size standard of $30 million. The product service code is Q512. 5. Provide a summary of the optician services that your firm provides. 6. Company DUNS number and confirmation that company is registered in System for Award Management, www.sam.gov. 7. Additional information and/or comments. It is the intent of the Government to issue a competitive single award Indefinite Delivery Indefinite Quantity (IDIQ) type solicitation after all market research has been completed.
