This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and solicitation for Commercial Items, in conjunction with FAR Part 13.5 for certain Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotation (RFQ). The solicitation document and incorporated revisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-06 (effective 05/06/2020). The associated North American Industrial Classification System (NAICS) code for this procurement is 541380, with a small business size standard of $16.5 million. Contractor is required to be actively registered in the System for Award Management (SAM) and Small Business Administration (SBA). SDVOSB & VOSB contractors are also required to be active in and shall be verified in Vendor Information Pages (VIP). Quotes will be considered non-compliant and rejected if the Contracting Officer is unable to verify active registration status. Michael E. DeBakey VA Medical Center, Houston, TX -Dental Service & Conroe Outpatient Dental Clinic is seeking a contractor to provide quarterly dental unit waterlines testing to ensure the water used to deliver dental care is of an optimal microbiologic quality. The Contractor shall provide all materials/ shipping packages to collect samples and ship timely to the Dental Coordinator for water sample collection in sterile collection containers. The samples will be shipped to their lab and tested according to established protocols of Certified Environment Testing Lab. Results shall be reported back to the Dental Service/Dental Coordinator; Chief of Dental Service and/or his/her designee, in a timely manner.
